The OT, PT, Nursing and support staff are amazing people who are top notch in their profession dedicated to the improvement of their patients health. They all deserve a 5 star rating. Where things fell apart was their Social Services Director not sure the root issue but first few times of inconsistent information was tolerable but unfortunately this wasn't an exception. Consistently changing or untrue information seemingly just to pass the buck, zero accountability. I feel bad for the rest of the crew who have to scramble to clean up her mess.

I've been here for a year as a patient and just like every skilled nursing facility you are going to have some bad aspects but usually overall the good far outweighs the bad. That's how I would describe life here. I feel bad for people who've lost loved ones and it does happen. I've lost two roommates here and neither was due to poor care or neglect in my opinion. Families want to believe someone should be responsible for their loved ones death even if they put them here at a frail old age with health issues. I see it everyday. Old people just get dumped here and their families forget about them in a matter of weeks. I'm young enough to observe and be objective. I think extra staff on duty would be great but I have to be realistic. There could never be enough staff on duty because as patients and or family we want instant attention. If that is what you require then I would suggest doing what generations have done for centuries. Keep your loved ones at home and love them everyday as you care for them. It's the most effective way to be positive that your loved ones are getting the attention you'd have them receive. Otherwise just keep paying strangers to care for them the way their families should.

I can give a first hand review of this place as I was a resident for a month after major back surgery. When I took my issues to the Administrative staff. They responded very professionally and Address all of my issues. My biggest issue was with their food but as this is an institutional facility they like other nursing homes follow a large dietary Provider's plan. As set forth by headquarters owners. But they did what they could to comfort me Personal thank to the Kitchen manager. They have a Extremely knowledgeable head nurse She is Literally a walking physician's desk reference. I sincerely appreciate long time 20+ years CNA Mary Jane If this place was so bad, as many has posted Why do There employees stay for more than 20 years? as I had noted at least 3 Most likely more, Even though this facility has a higher than normal Staff to patient head count taking care of the elderly. It's more than a full time job! The nurses and the CNA's Literally run to try to help The Patience. I witness more than once new hire interviews going on. For a few short days their air conditioner system system had problems. The head staff rented Very expensive equipment to make the patients comfortable. At no time, when I required and asked for ice water did i not received it. As a pass advanced EMT and An American Red Cross blood born pathogen instructor And one who had read The governmental requirements for the quarantine of the Covid Pandemic guidelines they were on top of it and very very strict as a matter of fact I was put into the quarantine unit for 2 weeks before before I could prove to them that I had my 2 shots. They apologize deeply and promptly put me in the proper room. from my heart which is surely Thank all that came to my Aid during my stay there. Thank you so much! "You are all doing God's work" bless you all. A special shout out to the staff in the physical rehabilitation division very knowledgeable and kind hearted. People on the outside, need to understand that those that work here have family also that they have to worry about In this time of a world wide pandemic. after curing for people that may have a deadly disease. They here are 1st responders also. I understand you may have for the 1st time in your life a very very deeply loved one That may be placed somewhere without 100% satisfaction. As for the facility it is extremely clean Again Thanks too long time employees and very well maintained. I have never been good at Remembering names but you know who you are, that helped me. And I profoundly thank you All...

DON’T PLACE YOUR FAMILY HERE. I really wish I could give no stars. This place is atrocious. I can’t believe they are still open the way they treat their patients is absolutely horrendous. I watched my uncle ask for assistance for over an hour with no response. I spoke to him long after leaving and he still been without pain medication. I honestly don’t understand how this place is still operating Especially with all of the complaints!

I was a patient in this hell-hole and being there was a total Nightmare that compounded my misery. Most Staff were uncaring. Many demeaned patients and were oblivious to their needs. So often, we had to beg for help. It was like torture when I had to urinate. There was usually an hour wait (sometimes longer) to be helped from the bed to use commode. Some patients couldn't endure the wait and urinated in their beds. I once sat over a feces filled commode trying not to vomit from the stench, and I yelled for help as staff walked past. Medicines were not checked with (me), a patient when administered. A nurse tried to give me medicine that had been discontinued. I saw that Covid -19 guidelines were not followed. Staff did not change gloves between patients. Most did not wear virus protection if supervisors weren't there. I was in a room with Patients who moaned in pain and begged for pain medication well past the scheduled time. Patients were crammed together in small rooms. There were no phones in rooms. There was no way to contact the outside. Food was terrible, and was often spoon fills of gray, malodorous slop. When I learned someone in the facility tested positive for Covid-19, I signed myself out AMA. This was one of the worst experiences of my life. I feel lucky to have come out alive without contracting Covid-19 or suffering from Post Traumatic Syndrome from this horrendous experience.

My brother stayed at the Woodland Skilled Nursing Facility in Woodland, CA for about 2 months. From everything he experienced and I dealt with as a family member- it was like a prison. It so happens he arrived there on his birthday. Upon speaking with one of the administrators I was told they like to make residents feel special on their birthdays. Therefore they would have some balloons in his room upon his arrival and make sure the staff acknowledged him on his special day. None of this transpired. Additionally, a few days after his arrival, I was informed he was moved to a new room. Supposedly my brother complained of a noise disturbance that would continue into the wee hours. Well, that was just a small fraction of the story. My brother needed to move due to being called racial slurs and receiving threats to his safety. This type of behavior is impermissible and anyone creating an unsafe and threatening environment to another resident should be relocated out of the facility. Furthermore, the phones to reach your loved ones were never functioning properly. If you were lucky enough to reach an employee they would tell you our phones have been malfunctioning for over 6 months. The staff suggested I call the administrative office to complain, as they had tried with zero success. Can you imagine going days without being able to check on your family member? When I reached out to the administrative office the gentlemen I spoke to appeared perplexed to the news and said he would look into the matter and get back in touch with me. I’m still waiting on his call and my brother has been discharged. To add, I received a call from the office to inform me that my brother was indirectly exposed, to someone who tested positive to COVID-19. As if his stay couldn’t get any worse. He was now quarantined to his room for 14 days. I couldn't even envision what my brother was feeling or dealing with. On top of that, I was even more distressed that I wouldn’t be able to reach him more frequently, with the poor phone systems. I can’t tell you how defenseless I felt knowing that my brother was dealing with recuperating from a horrible accident and he lives in California with absolutely zero family members there to visit him. I thought it was my lucky day when I finally reached a nurse that happened to work on my brother’s floor. Well, that luck ran out instantly. She was so extremely rude, informing me that for her to connect me with my brother, they would have to put on protective gear since he was in quarantine. She nor the other nurses had the time to do that nor were family members speaking with the residents a top priority. I told her I hadn’t spoken to my brother since I was told of his indirect exposure to COVID-19 and I wanted to check on him. She then brutally replied I’m very busy right now, I need to go and I can’t promise you anything and hung up. How is someone with zero amount of humanity, patience, and kindness working as a nurse? My strong suggestion is to choose a different nursing/rehabilitation center. STAY AWAY from this dreadful and disheartening place. This place is not worth the 4 stars they receive on some of the sites I viewed.

My sister was in there and the way they treat patients is not acceptable!! I would guess 2 out of 10 nurses are there that do their job. I'm sure that they don't get paid nearly enough, but not a reason to take it out on the care they are supposed to give the patients!! Letting patient's wait 2hrs or more on answering the call button. Leaving the patients soiled with feces leaking on them from colostomy bag over full or just plain leaking!!. The CNA's are more attentive and do the jobs that the RN's are supposed to do. I wouldn't put my worst enemy in here!! If you have no choice, then keep a good eye on your loved ones and pray they don't have to be in there long.

Terrible nursing home. They lack sympathy, urgency & detailed communication. Since my grandfather arrived from the hospital due to a head injury, we were told we couldn’t take him home since if he exerted any type of force or fell one more time we would loose him, due to his head surgery and broken vertebrae that was pinching his spinal cord. - So it was critical for him to be overseen 24/7 while he healed by a professional staff. This place tried to provide as little information as possible to cover their lack of attention and profit driven strategy of patients to medical staff assistance ratio. Family members were called by grandfather at the time that he wanted to rotate his body, since he was tired of being in the same position for hours and they couldn’t help him until the next day since they were short staffed on the weekends. My aunt went to the nursing home Immediately and she was not allowed to go in for unknown reasons. When she finally went in they had my grandfather on the ground as emergency services were providing CPR. He passed away last night due what seems to be lack of resources, protocol constraints/untrained emergency situations and high possibility of medical negligence since he called family members to help him as a worse case scenario. His root cause of death is currently under investigation by the woodland sheriffs department.
