Yes I am writing in reference to moving my 92 year old mother from encompass for additional rehab. About 5 years ago my mother had a broke hip somewhere around that time she had to go to encompass and after her time up there she was moved to westside terrace and did remarkable. We even paid two additional weeks then starting in Nov 1 2021 she could not get up out of her wheelchair and took her to southeast and three days later she went to encompass and stayed 10 days. She was doing good and getting up out of wheelchair with minimal assistance so we talked and we thought if she could go to westside terrace with additional rehab it would be good for her. Well we transferred on a Monday and during that first week I had requested for her to go to beauty shop they put her off numerous times I had to see charge nurse and they were not as at tentative before I should have know something . I went down there one day and her hair was braided and my mother never had her hair braided had to complain went back down there and again it was braided then she tried to get up and fell on floor.had terrible bruises on her head and then all things changed and went down hill . They sent my mothers to flowers hospital thought she might have a brain bleed . The lady in the room told me my mama layer on floor for awhile. No one seemed concern then they tried to tell me she didn’t have a room. She went back then they tried to tell me she needed additional rehab and this and that. I told them some nurse had my mama calling me repeatedly that weekend but I was down there Monday morning to get her out. And if since my mother went down hill by January 1 she was bedridden and doctor was ready to call hospice in. I asked repeatedly for records from westside terrace and because I did not have poa would not give them to me but I checked her in and out was there many days and went to fliers to meet her being transported. The main people were rude and not caring and staff on floor Was not attentive. I would never recommend to put any relative there . It has been a roller coaster ride from the day my mama fell and s sad

Every employee I have encountered has been nice and helpful. I will say no one has ever been rude to me. It is almost impossible at times to get someone to answer the phone. The lack of employees in all areas is disturbing. When I have left a message I may or may not get called back.There has been very few activities to encourage the residents during this covid time of one year and a half. I know for a fact there is more that could have been done. Without the residents there would be no Westside. The goal of a long-term nursing facility should be to make it as much as a home as possible filled with love and fun activities. I believe the above issues are due to employees expected to do to many tasks. I believe the employees are overwhelmed with to many responsibilities.

We sent my grandmother to this Rehab center. Before she went in this awful place she could walk, move her hands, and use her legs. After coming out of this place she wasn’t able to do anything. She had turned into a vegetable. She had a severe fracture on her neck, which caused her to stop breathing at one point. She has since had surgery on her neck because of her falls, and is currently in Flowers Hospital. She fell several times over there. My grandpa and I went to see her one day and found her laying on the floor. IF YOU LOVE SOMEONE DO NOT SEND THEM TO THIS REHAB!

Find better for your loved ones. Disorganized and ill equipped. Clueless management personnel, no care planning. Broken down, outdated facility with bad food. Staff openly berate and neglect helpless patients who depend on them for care. Avoid this place.

ABSOLUTELY POOR COMMUNICATION! Visitation Policies Absolutely DO NOT coincide with CMS or CDC Federal OR STATE GUIDELINES! No communication with how my FATHER is doing with his PT/rehab. I was not able to see my grandfather because I am not old enough and they said it was because of COVID but younger people have a less chance of getting COVID than anyone. Also you can only visit 30 minutes a day and they don’t have visiting on Sunday.

My dad is there and we couldn't reach him because his cell phone wasn't ringing. Asia took the time to look at his phone and figure out what was wrong. Thank you Asia
