Kaiser should stop referring patients here. The staff is not friendly and discourages long visitations. My father has Parkinson's and was transferred to this facility for rehab after being hospitalized for sepsis. The Dr.'s orders were not followed most of the time and family had to constantly be on top of everything otherwise it wouldn't get done. As another reviewer noted, when a patient arrives they need to be evaluated by a therapist before staff are allowed to assist patients to use the restroom. We arrived at the facility at 5pm so there wasn't anyone available to evaluate him until the following afternoon. We called a nurse to provide a bed pan and waited 45 minutes, and still nothing. We finally stood him up and transferred him to the wheelchair to take him to the restroom ourselves. This lack of attention went on for the duration of his 8 day stay. His medications didn't arrive until the following afternoon causing him to miss several doses of medication and crucial IV antibiotics. Several times we arrived at 8am (Nurses don't like anyone there before 9am) to find my father soiled with a full catheter bag and freezing as he was unable to get out of bed on his own for the first few days. We are fortunate to be able to have a family member there with my dad from 8am-8pm everyday of his stay. Although, much to the staff's dislike. Many patients are heard yelling for help or whaling in pain for hours, and they were just ignored by staff. Another example of negligence was that as he was being discharged the attending nurse unnecessarily removed his catheter stating that it was Dr.'s orders. This was the 2nd time during his stay that the catheter was mistakenly removed. We wound up in the ER again 2 days after leaving the facility with a blood infection probably due to the unsanitary conditions and carelessness of the care staff. The facility is run down, the beds are old, the linens tattered and stained, the paint on the walls is chipping, the floors and restrooms are dirty and the door knobs and restroom fixtures don't even meet ADA requirements. I'm not sure how they get away with it. The physical therapists were very good. He did make excellent progress in his physical strength recovery due to their hard work.

We were here on 5-19-2022 and left on5-20-2022. This place deserves 0 stars. This place is the worst place to be at. The staff doesn’t care about you. The safety of the patients is not their #1 priority. This Place is so out dated the beds don’t have rails on them so its very easy to fall off the beds. You have to make an appointment to come visit your loved one, which to me is very odd, but after spending the day there visiting my wife now I know why you have to. It’s because they want to hide the fact that they don’t help your loved one’s. This place is very unprofessional. If you call for the nurse its take them about 1 hour to come to you when they are just sitting at the front desk doing nothing. The lady in the room in the next bed called to be changed and was sitting in a soiled diaper for 1 hour before someone came to change her. Read the reviews, everything all the other people say is so true, I saw it first hand.

They are not passionate about the well being of the patients. The follow up takes 99 to 120 day. They never have any answers with the exception of I,ll get back to you. They are all in a meeting. The doctor ordered it. They always apologize with no changed behavior. The staff turnover is alarming. There documentation and follow up is beyond the standards of quality patient care. They take your money throw them in a room and do not properly take care of the patient. Take you power back for you loved one an report neglect or failure to respond in a timely matter . Report them to Joint Commission Ombudsman for the City of West Covina or the Public Health Department. When they know better they will do better.

My dad was recently placed in this facility, when I went to visit my dad I asked my fiancée the time, we were there an hour and no one came to check on him. Also, the temperature in his room was 87° which is to hot for any patient to be comfortable. They did not do anything to rectify the problem only ignored the problem. This is a second rate facility, if it had not been a holiday he would be out of there. So I suggest they read some these reviews and act on them.

The nurses and staff did a good job I was there for 2 months . Michelle, Jason. and Ana and Victor did a good job. The CNA's did a good job. Daniel, Olivia and Elvia and Dyan and Lina and one more guy did a good job. My problem was 3 the roommates I had were total A holes. Also sadly they lost all my clothes from my room including my Steelers blanket. They made me leave my clothes. Since I got Covid from being there. The idiots probably ripped me off no accountability. I can't make a PD report since the DA allows small amounts of stealing things. It's obviously under 1K so I am F ed. Also no security officers so my report is mixed.

One of the best nursing homes/rehabilitation centers I have visited and spent time. My mother was admitted and stayed for almost two weeks for a followed up rehab care as a result of a stroke. I found the staff very polite, attentive and friendly. They always responded to my requests and tried their hardest to meet her needs. Above all, they seem to sincerely care about their patients, and long term residents. The facility is very clean although it needs some cosmetic facelift. I sincerely recommend it.
