My hubby just spent 3 weeks here.. It was a great experience..I was concerned about placing him in a SNF.. I voiced my concerns and he was treated wonderfully..I would not hesitate anyone in my family going there. He was in Sante of Chandler for 3 days and I pulled him out in the middle of the night.. Absolutely horrible..there I did have one issue at Wellsprings with a rude nurse.. I complained about her.. she was let go from working there from the agency . Shout out to PT..Tyson OT.. Krystaine Speech..Julie And our CNA Tiffany.. My prayer is that he never has to go back, but if he does it will definitely be there.. He is a double stroke survivor and numerous complications.. He is not mobile and in a wheelchair but working on walking again.. Thank You Debbie and Curt Hicks

Well, unfortunately there isn’t an option lower than one star. Questions: 1. Do you care about your loved ones? ✅ 2. Do you have their safety in mind? ✅ 3. Do you want to sleep at night knowing they’re being taken care of through kindness and respect? ✅ If you answered yes to ALL of these questions, it’s best to find another location that’s suitable.The center should have regular visits from those who oversee such places to protect the rights of patients. It’s a major concern to think that they are allowed to stay up and running after so many complaints.

I have been to several acute and skilled nursing rehabilitation center and by far Wellsprings is the best. There staff is amazing, the center is very clean and no nasty smell like most have. You can turn on the call light and you will have on average someone in your room within minutes the longest I have had to wait was around 5 minutes.They allow your K9's (with updated shot records) to visit you. The food is actually good. I would definitely highly recommend that any of my family and friends come here if they need a skilled nursing rehabilitation center to stay in. I know they will get the best care possible.

My dad was admitted here and we took him home within 24 hours. He had pushed the call button twice over night for help, no one came. In the morning when someone finally came, no one apologized or said anything. Thankfully he was ok and it was a non emergency but what if he had fallen down? What if no one noticed until morning. They seem to have a total lack of empathy for him. I would not recommend taking a loved one here with the high chance of neglect.

My husband was placed here by the hospital after he was diagnosed with viral meningitis complicated by covid. He was having cognitive issues therefore having difficulty at night and needed assistance with going to the bathroom. He apparently soiled himself and was left alone developing a rash we're still fighting. He had trouble feeding himself, they left him alone so he didn't eat. The food was sparse and tastless. The assistant director was the rudest woman who basically told me that they weren't equipped to take care of him and tried to transfer him to another facility for people with "behavioral problems". When we checked that facility it was a lock down for psychiatric patients. She insisted we take my husband out of there then became belligerent when we began packing his things to leave. This entire ordeal was difficult enough but my husband is still suffering from the affects of being here. The facility and furnishings are in horrible repair and smell nasty. If you love your family member, don't send them here.

My wife was taken to Wellsprings for therapy and recovery after being in the hospital for a month after being on a ventilator with COVID. The therapy and care she had was outstanding. The staff was very compassionate and willing to help when she needed it. Sure they weren't in her room all the time and she had to wait a little for help sometimes when she called them, but that has to be expected even at home with my support. The facility wasn't new but it had all the equipment and material needed for her. My father was put in a different facility a few years back, that was rated higher then Wellsprings but Wellsprings was far better and friendlier than the more expensive and better rated facility my father was in. I would recommend Wellsprings for therapy and recovery. I really appreciate and thank them for the help and support that they provided. My wife is well on her way to recovery.

This place is horrible. They had the AC turned off on my father who can not get up on his own to turn it on or off. My father was asking them for assistance and it took them more than 15 minutes to show up. He fell twice at this place and none of them told us that that happened we found out from my father. We kept asking them to transfer him back to the hospital because he was getting worse at this place and not better and they kept refusing saying they wanted to keep him the full three weeks that insurance covered him for although he and us requested he be removed and put back in the hospital. It wasn’t until we started to make stern threats that we will call 9-1-1 to get him help that they actually helped. If you really do love your loved one you would stay away from this place.

My mother spent several weeks in Wellsprings after hospitalization for chronic pulmonary and cardiac issues. It is not the place for someone with such issues to recover and strengthen themselves before returning home and Wellsprings should not have accepted my mother as a patient. The administration was in flux at the time - I believe that my mother’s case manager and perhaps someone else were let go while she was there - and it showed in staffing and attention. The therapists, nurses and aides themselves were warm and competent but there were not nearly enough of them to go around. For weeks, with the exception of an hour or two of therapy each day, my mother sat alone in her room, and was not taken on walks or outside, which was important to her recovery and to slow the onset of edema. She regularly waited for 40 minutes and longer for someone to help her to the bathroom. I understand that Covid required facilities to make adjustments, but even accounting for that, I feel as though Wellsprings simply took the insurance reimbursement money and did very little in exchange for it. She would have been better off at home with therapists brought in. It is shameful.
