My mom is said the last time I saw her. Her hair was nodded and the room was so hot. It was a 100°. I had to go by her a fan . And I have to call the paramedics. We are working on getting her out as soon as possible. Highly do not recommend this place. Somebody just needs to contact the Health department need to be Shut down by the state.

Poorly kept facility, management is rude and does not return calls. My grandson has been there for a few months and we have never seen him out of the room. They have nothing for the patients to do but smoke cigarets and marijuana in front of the building. We have witnessed violent behavior from patients throwing things at the nurses and attacking staff. My husband was approached by a very drunk man that we do believe lives there in the building, patients block hallways playing loud music disturbing the people that are resting. The 5 nurses were sitting at the desk not helping people or trying to calm the patients down, a man was urinating all over the social room floor and the staff in the office were yelling for him to leave. We don not recommend anyone allow their family members to be sent to this place. We will be getting our grandson out of this place as soon as possible. Please make sure to go and visit any place like this before blindly allowing your loved ones be sent.

This place is absolutely filthy!! I'm almost positive they do not have housekeeping. Unsanitary, throw up on the walls, bathroom floor imbedded with dirt, urine stains, and feces, absolutely disgusting!! Stained linens, and thick thick dust and dirty build up in the heating vents. I personally spent 4 hours cleaning my brother's room and I am still not finished. Here they collect every residents, and patients government income.... What they use it for is a mystery to me! Because it's not attentive care workers or sanitation services let alone housekeepers! Someone from the state needs to come and thoroughly inspect this place immediately!!!

If you care about your family don’t bring them here. Take it from someone who works there and have been there 8 hours a day. You can never get ahold of anyone Lauri is either in a meeting or hiding in her office ignoring residents until its benefits her. I literally got hit by a car and the first thing they said to me is my position was filled and still won’t give me my money. Also they can’t even communicate to each other at all and majority of the resident are high or drunk or getting drugs from staff members no rehabilitation is getting made .

Pearl Street is the best facility in this area providing many levels of care. Very clean, organized, friendly facility with loving, caring staff. Their staff is phenomenal, with a high retention rate, so the continuity of care is always consistent. They genuinely care about their residents, as is reflected in their topnotch nursing and rehabilitation efforts, as well as the interest they take in the unique needs of each individual. Family members can always rest assured that their loved ones are taken care of with the highest, upmost respect.

10.27.2021 - Apparently they delete reviews they don't want people to see - as my original review is right below - I will continue to post this on your site until I am no longer breathing if need be ... people NEED to be aware of what type of facility this is!!! You didn't take my concerns regarding my mother serious, so now you can deal with me ensuring this review stays for everyone to see! As they very well deserve to!! Absolute trash! The entire facility is filthy! My mother was in this facility for awhile, worst decision we ever made on our part. She had fallen down, broken her hip and it wasn’t noticed until 3 day’s later! She ended up at Swedish across the street where there they found she not only had broken her hip, but also had a bad case of pneumonia ... straight to ICU from there, where she spent over a month in there and almost died! Thank God for the Dr’s and staff at Swedish! I wouldn’t recommend this facility to my worst enemy! Beware! If you truly care for your loved one, do NOT put them in the care of this facility! Fair warning.

This is the truly most disgusting nursing home I’ve ever seen in my life. They literally live in their body fluids and cigarette smoke. Elevation Health and Rehab is a homeless camp with an address.

This place is by far one of the worst. They have sent us home with my mother with CARDS I mean CARDS of medication. So much so that we had to worry about being pulled over or be questioned why we had them. Then when we brought my mom back we were questioned why we took them? They also were over dosing her with her meds that sent her to saint Joseph in the ICU because she was septic and was being slowly poisoned by her medication. They discharged her right after they found out about her prognosis so they could clean their hands of thier faults. I’d say stay away and I hope thier staff and administration get the same treatment in their life as they were giving to their clients. I dearly must say. EFF YOU PEARL STREET!!!!!!!!
