This place is filthy. They NEVER answer the phone, NEVER!!! My dad's leg was oozing with water and blood, and they allowed it to soak the top part of his sock, not changing it. The bottom of his pants leg was wet from his wounds. He somehow acquired more wounds on his leg than he previously had. He had a string of hair and specks of dirt in his WET and OPEN wounds. When asked if a CNA could take him back to his room (without her noticing me), she replied to the charge nurse, saying "really?, really"? Then she saw me and tried to play it off. When she placed him on his bed, she placed his leg with obvious wet and open wounds directly on top of his dirty clothes that were laying on the bed, and then proceeded to toss a blanket right on top of his leg that had wet and open wounds. I noticed a habit with staff that they kept referring to him as a "she," as if they didn't know their pronouns. If you have any ounce of love for your love ones I BEG YOU PLEASE DO NOT SEND THEM HERE. I've called the state and reported them. I don't know how this place is still open. As a social worker observing, I promise you that these people care nothing about the people they serve.

My father was transferred to this location on 11 January 2024, I had him out by January 12, 2024 5 p.m., this is a complete dump, outdated building from inside and out, nurses, not attentive on phones, One nurse kept on, referring him as a "she",doors of rooms beaten up, Put brief on as soon as my dad entered who was never in briefs, Lighting in room, never worked, whole facilities, smelled of urine and God knows other things, the night of the 11th I went home I could not sleep because I kept on thinking about my dad and this horrible nursing center,I was told it was a five star Center from the hospital where my dad was discharged from,when I saw this, I said this place is NOT five stars, it deserves a -0 * my personal opinion, I had my dad out of that place by 5 PM on 12 January, I would never recommend this place for your loved one. I wouldn't even recommend it for my enemy,. Once I knew, and witnessed my father being transferred to another facility, I came here to give my review.. Such a horrible, horrible place!!

I would have left no stars but the reviews make you at least leave one! My aunt was at their facility and left by ambulance to the hospital on Easter April 9th, 2023. A family member went back to the rehabilitation on Thursday April 13th, 2023 center for her belongings and the social worker could not find anything. Her cross with mother's ashes were inside, a book with passwords for her phone was there and a fitted shrink sock for her nub in which she had her leg amputated is why she was there. I called on Thursday and left a message, in which no one called back, I called Friday and did the same and no call back. Mind you the receptionist kept sending me to Taylor who was the social worker manager in charge and never received a call. I spoke with someone on Thursday who stated before they put a new patient in the beds they clear things out of the drawers and have an inventory sheet. In which obviously they didn't do. A patient replaced that bed my aunt was once in within 4 days. This place workers would sometimes have attitudes. Some were good and well respectful. Others acted as if they hated their jobs. Now they have no answers as to where my aunts personal belonging are. Don' Send your love ones there! Worst Rehab! They have no solutions to any of the issues!

WHNR staff and team are amazing! Staring off with both receptionist who are always so welcoming! Always with a big smile and with such positive energy. Taylor SSD has been amazing! Taylor and her team in Social services go above and beyond for patients. I witness all the help and support they provided to my patient as well. The nurses and CNAs are truly wonderful. Janet LVN is so caring and dedicated to her patients. She was always so kind and helpful at all times. Gloria CNA is also wonderful. My patient had nothing but great things to say about Gloria. Last but not least, Stephanie In Admissions, Arleeen and Teresa are extraordinary at what they do! Thank you WHNR team for all you do and continue to do for patients!

I recently visited Wagner Heights as I am looking for a facility for my mom nearby. I really appreciated how enthusiastic and helpful the staff were. The physical therapy room was very bright and open. While visiting, they had entertainment for the residents that was great and even I enjoyed! I would recommend!

This place has roaches. The staff admitted they know about it. I removed my mother immediately and basically the staff tried to intimidate me into leaving her there. The supervisor on 11/14/22 in the so called VIP area said she didn't see any roaches but there were roach baits all over my mothers room that were not there originally. They wouldn't release her meds even though they don't belong to the facility.

The social services of Wagner Heights are not diligently doing their job in the field of providing social services especially on indigent elderly and on those who do not have any idea what to do or where to go. They are neglectful of their responsibilities. My mother is an indigent elderly and when I spoke with John who was with the social service, I was very outright candid that no one among the children are capable financially to take care of my mother’s need and I have no idea what to do to get her assistance and to compound the problem, I live out of state. She is on kidney dialysis three times a week. I asked John about the arrangement for this and his response was that it’s the family or the patient who has to make these kinds arrangement. He knew at the get go that I don’t what to do as I’m also incapable of helping out my mother. On the day that she was discharge, she had to pay for the transportation services despite the fact that John knows that we all have very little money to spare. Again, as a person who works in social services, he does not seem to be inclined to help and find resources that can help my mother. The following day after my mother got discharged, she was once again brought to the emergency department because she was unable to have her dialysis that morning as she was supposed to have. And she was told by the social worker at sutter hospital where she was brought that evening that someone from the social service from Wagner heights should’ve help facilitate to find resources that will ensure that she doesn’t miss her kidney dialysis. What would’ve happened if my mother died because of kidney failure because she didn’t get her dialysis? What will the social services of Wagner heights do? Shrug their shoulders and say, “oh well, too bad, so sad.” This is negligence on the part of the social services because they neglected to make sure that my mother’s post discharged need are arranged. His idea of a discharge plan is to just make sure that she will pay private transportation and that she gets to her apartment. Beyond that, it’s pretty much on the shoulder of an elderly indigent patient. Do not send your elderly in this facility. They especially don’t care if you are indigent and that you lack the ability to find resources to augment or facilitate your medical needs and post discharge needs. You are on your own.

I recently visited WHNR and was impressed. The front lobby was inviting and the receptionist was very nice. Walking the halls, I saw people who looked well and cared for. Staff were pleasant and helpful. Well done Wagner!
