Transitions Chronic Care

BrightStar Care of Northern Colorado recently started working together with Transitions and it has been nothing but a wonderful experience. The RN that we have been working with, Paige has gone above and beyond in every way possible to coordinate care with us and voluntarily aid in communication with providers. She is so responsive, communicative and has made working together a fluid process. Additionally, Paige has a great attitude and very clearly cares about being an advocate for her clients! We hope to work alongside Transitions much more!

Transitions Healthcare was a Godsend. They helped me with my 91 year old mother-in-law, providing excellent, excellent care, understanding, and compassion! Dr. Mark is the most compassionate, intelligent, and understanding doctor I have ever met. He has the most beautiful mannerisms when speaking with the elderly ( my mother-in-law) demonstrating understanding while ensuring their dignity. Bravo to the staff and the doctor at Transitions!

Transitions Healthcare recently provided me with in-home Physical Therapy for a few weeks after my spinal surgery. The physical therapist, Lauren, was extremely helpful with helping me gain back strength and mobility with safe, gentle and effective exercises in line with the precautions advised by my surgeon. In addition, Transitions sent an Occupational Therapist out shortly after I got home from the hospital to check that my house was safe and functional for my post-operative recovery. She offered good tips for easy, temporary adjustments I could make while my mobility was limited. I highly recommend Transitions for post operative, in-home care.

I'm have been happy with the services you do. Page is wonderful to work with. She has ways made me feel safe and comfortable in what I have need. She answered any questions I may have. Thank you for very thing you all do.
