The Meadows Post Acute

My mother’s experience at this rehabilitation center was disappointing and concerning. Unfortunately, I observed neglectful behavior towards patients, especially the elderly and patients without advocacy. I became aware of falsification of medical records when I asked to see her prescription medication, which was still in a sealed package. It was evident that her declining health was a direct result of the nurses not giving her the medicine, but logging and signing the medication records. How criminal and dangerous! The most troubling aspect is that the administration and supervising nurses are fully aware of the issue. If you or a family member are admitted at The Meadows, ensure that you are constantly vigilant that the medications are administered as prescribed. The overall atmosphere felt lacking in care and professionalism. I strongly advise others to consider alternative options for rehabilitation services.

The Meadows Post Acute in Panorama City is a place of healing and improving QoL (Quality of Life). It is run by a very efficient administrator, Khristine B, who personally interacts with families and ensures that the patient receives every thing to make their stay comfortable. The Dir of Nursing, Dir of Rehab, Dir of Staff development, Dir of Infection Prevention Dir Of Admissions, Social Workers, charge nurses, support staff, Activities Supervisor, Kitchen Supervisor, Facilities and ancillary staff are cheerful, compassionate, polite and very prompt. This team is fantastic - we always felt welcome during visiting hours. A special thanks to the evening and night shift - their personal touch made a vast difference. During rare down time, they even sat down and talked to patients and family. We never felt alone during our stay. Rather we felt surrounded by close friends. This team ensured that my dad received the best care possible; recovered; and was healthier leaving the facility than when he came in. Sincere thanks to the amazing staff of Meadows, Panorama City.

I am a licensed psychologist that has been working here for 3 years I see almost all the residents. I am blessed to work here, I work with 14 other facilities this is by far the best. Sandra the DON is amazing everytime I address a medical and psychological issue she is on top of it with care and compassion. The administrator Christine knows all the residents and personally visits everyone to address any concerns. Brenda and Mark the SW are on top of the facility checking in with the residents needs all the time and very attentive. They really care for the needs of every resident. The activities director Ingrid knows every resident and tries to make them feel safe and busy with activities. This place is the best which makes my job at the facility easy. The facility is always clean and comfortable. The staff here work amazing as a team. I would trust my family here.

Very bad experience . The front desk wasn't helpful at all she was very rude . And also after a few times calling i got the director of staff development , DSD . And make appointment with him . I was on time . After I did my application and ask 4 interview , he said he has meeting . What's the point making appointment ? He doesn't know that's 4 sure and the facility rely on him . Very sad . 😞😞😞

The Meadows had many problems when Sheri Reyes was the Administrator but as soon as they brought in a new one it's becoming a GREAT place to live and rehabilitate.I recommend it for all patients young and old.

Great facility with top notch care providers… My elderly friend received excellent care at The Meadows Post Acute for the months he was there. Everyone involved was extremely communicative and kept me up to date my friend’s health. The staff was very nice and genuinely seemed to care about the patients and those who came to visit. One thing that particularly impressed me about Meadows was that whenever I would call, if my friend did not pick up his phone, someone would actually go to his room to see why not and try to get him on the phone. I have visited my friend at all different hours, and they are consistent. Facility was clean and tidy. If any of my Family members need nursing care, I will definitely have them come here!

I just met with my son's care team and can only say that a boulder was removed from my shoulders. They are so caring, empathetic and warm! They answered all of my questions and never made me feel rushed. The grounds are lovely and welcoming. If my son must be in a medical care facility, i thank heaven that he's at The Meadows.

Staff has been incredible in taking care patients. Facility is clean proper and very friendly. The main focus of physical therapy and getting back to health makes it easy to accomplish at The Meadows Post-acute care. I feel safe with family members that have surgeries or need rehabilitation to come here!
