The Blossoms at Van Buren Rehab & Nursing Center

Wonderful caring and helpful staff. Everyone from the cleaning crew to the administrator. The nurses are short staffed but do their best to put a smile on their faces for those that are unable to care for themselves. The facility is clean and has no odors. I’m glad to know they are taking care of my aunt.I appreciate all that they do for her.

If I could give this facility zero stars, I would. Please don’t send your family here. This is bottom of the barrel longterm care. Anywhere else would be better!

Second time we have used Crawford Co Rehab to prepare my Dad to return home from a hospital stay. First time in 2018, was a good experience but in May of 2020 during COVID it was not good. He transferred from Baptist Hospital in Fort Smith after recovering from a blood infection straight to Crawford Rehab for what was to be his 21 days to gain his mobility back to return home. Suddenly after 10 days we were told they were releasing him with no explanation. Remember COVID restrictions had not allowed us to see him during his hospital and rehab stay. When my mom picked him up from Crawford Rehab he complained of severe pain but couldn’t tell her were it was coming from. He returned to Baptist Hospital within 8 hours where they discovered he had 2 broken vertebrae. He then endured more weeks in Baptist before my mother was allowed to stay with him his last week before returning home to see his family before he passed away. Make your own mind up but I cannot recommend this facility for your loved one. ❤️ (picture was taken while he was in Crawford a few days before they told us to pick him up)

had bed bugs in my grandpas bed and then moved him to another room. That is something that needs to be taken care of. i wouldn't put anyone you love in this place. they don't take care of you and we should have put him in fort smith.

I’ve worked at this facility for 10 years now. It has always been a joy and a pleasure being part of the care of the residents at Crawford Healthcare and Rehab. When it came to the dreaded time that my grandmother was no longer able to live alone and needed 24 hour care, I knew that she would get the best care at Crawford Healthcare and she did! She made so many new friends in her short time there. She was loved. Crawford has such a dedicated therapy team, whether it’s to help long term care residents stay active and moving, or to help the short term residents who are not able to return to their homes right after a hospital stay get back on their feet and return home fast. Every resident becomes part of our family at Crawford, and we all want the best care for your family. This is where you will get that care!

Just wanted to back up most of the other reviews here about the lack of care at this facility. Patient's are supposed to be able to come here to recover from surgeries and regain independence? Terrible. Do not send your loved one here.

This facility has done a complete 360 ,I was visiting today and they have lots of specialized trained very friendly staff.The halls smelled really good and I noticed the staff were very attentive to the needs of the residents. I was very impressed with the cleanliness of the facility as well as the highly trained attentive staff. I have been a certified nursing assistant assistant for 26 years and I would recommend this facility to family.
