One of the case managers at the hospital where my grandfather was admitted recommended The Avenues, and I couldn’t agree more! From the moment we walked in, the front desk staff was incredibly welcoming, which made a great first impression. All the staff members were so friendly, and it was obvious they genuinely enjoyed working there, which created a positive and uplifting atmosphere. The care my grandfather received was outstanding. The CNAs and nurses were always prompt, attentive, and compassionate, ensuring he was comfortable and well cared for at all times. I also appreciated the variety of social activities they offered to keep residents engaged and entertained during their stay. It was great to see how they encouraged family involvement, making us feel like an important part of his recovery process. The Avenues goes above and beyond to provide excellent care and a supportive environment. If you’re looking for a top-notch place for geriatric rehab, I highly recommend The Avenues. The Avenues transitional care was such an amazing place for my family member. I have been working in the home health field for more than 10 years now and I have to say The Avenues are one of me top SNFs to work with to help patient transition to home. I work closely with the social service department with patient's discharge. Kate, Cathy, Arielle, and Nerissa are all very professional and fast at communicating with me regarding patient discharge plans. Andrew, the case manager is very on top of getting authorization needed depending on patient's insurances to ensure it is approved so we can start our services in a timely manner. I have visited the facility many times and I've seen how attentive staff member's are to patient's needs. When my family and friends ask me for recommendations for SNF in San Francisco, I will always tell them how great The Avenues are in all aspects!
