This nursing home is a nightmare and should be shut down immediately. My mother was admitted to regain muscle strength and learn to walk again, but their negligence left her bedridden most of the time. They consistently left her in soiled diapers, resulting in a severe bedsore that went untreated. Communication with the staff was nearly impossible—they rarely answered the phone. When it was time for her discharge, my mother was still unable to walk, and I had to carry her out myself. Shockingly, she was still in a filthy diaper. When I attempted to bring her back inside to use the bathroom, they refused to let her in, citing that she was discharged and we had signed the release forms. They literally shut the door in our faces. This facility is beyond incompetent and borders on abusive. Avoid this place at all costs. It’s a disgrace and should be shut down for the safety of all its residents.

These high ratings are all a farce. The only employees worth anything are CNA Cynthia and CNA Mary. My real review had be cut down significantly but here’s what I could fit: I had the misfortune of being admitted here on a Friday evening and by Monday evening I was back in the ER. This institution lacks decorum, professionalism, tact, dignity, and empathy. They are also full of thieves. I am a woundcare patient and they had me sharing a room with someone who had MRSA. The other roommate was unconscious 24/7 and would choke on her own phlegm day and night. Roommate 1 would have to literally argue with the staff to get them to suction it because the poor lady couldn't advocate for herself. She was also only changed once a day at 4am despite soiling her diaper several times throughout the day. She was also fed via feeding tube and the specific instructions to prepare her food was to use distilled water, but they use sink water to make it. This is textbook NEGLECT. I was deprived of my pain medicine I rely on to ease the pain of my bone disease because "they didn't get the order" despite it being on my hospital discharge paperwork and they got all the other orders. My surgical incisions ripped open and they denied my request to go back to the hospital. The wound nurse is stubborn and overly confident in her capabilities. I reached out to my surgeon and he requested I go back to the hospital asap. It turned out that I did in fact need another surgery. She also refused to clean my wounds with rx wound wash that I had. she wasn't gentle with my woundcare she pushed hard and was just trying to rush through. The ignorance is astounding They use the same blood pressure cuff on everyone without sanitizing if before using it on another patient. The curtains are dusty, and the rooms are kept at 80 degrees. If pee gets on your sheets they won't change it, you have to request it. Also the overnight cna won't even wipe you after you've been sitting in pee Their website advertises iv therapy but they deny patients who need it (roommate needed iv antibiotics for her mrsa and they denied her). When leaving I took as many personal items as I could and told them I would be having someone to come get them. it's clear that the facility stole them. the items I left was over $200 worth of woundcare supplies and wipes (which I know they steal based on my experience on night one). My friends went in to collect my items over the weekend, which they were notified about, and when they got there they just gave them a vase with roses that I left. When I told them there was more they said they'd have to come back Monday. Monday morning I call and I'm told to call back at 9 to talk to social services. I call back and social services tells me my stuff should have been ready over the weekend and I explained what happened and ahead of time told me to call back at 1:30. I call back at 1:30 and she still has no idea and says she will call me back. I don't hear from her and my friends are going back in today to get my stolen belongings so I call at 4:30, no one answers. So I call with an anonymous number and get hung up on by some lady who answered the phone with "hello" instead of the proper greeting and when I asked is this San Jose health and wellness and she said "what you mean?" After asking her to transfer me to social work she asked my name and when I told her she hung up on me. Ten minutes later I called again and spoke to another person who told me "the social worker is busy" so I left my number and never got a call back. My friends went in to get my stuff and the lady literally turned around and walked away mid conversation. They lied to my friends faces about my stuff and I've been trying to call again and they refuse to answer. They also don't answer emails. It's unacceptable and unprofessional. No one in administration has an ounce of dignity or honesty. The CNA that was with me my last evening is a lying coward. This place needs to be shut down for inhumane treatment of those poor people in there that can't advocate for themselves

The staff here are focus and on top of their daily responsibility. My father is admitted here twice by a good recommendation by our family doctor, Dr. Guanson. My father is healing from his surgery and prostate cancer. Today he is well and getting better so he can be release safely home.

This review was long over due. I only decided to write it because my dad was going to be sent here and I was like hell no! My mother in law was going there back in 2020 when the pandemic was going on. They didn't let family in to see the patients but yet someone that works there brought covid and my mother in law passed away. It's so ridiculous that they were so strict for them not to be careful. If the patients didn't go out and no family came in who else brought it? They should had been more careful knowing people are sick. I DO NOT RECOMMEND THIS PLACE AT ALL! If I could give 0 stars I would.

My mom was transferred there from O'Conner Hospital after her hip surgery. I couldn't go inside because of covid-- but I insisted on seeing her every day through the sliding doors. I was told they had extra staff for Alzheimer's patients. Nope. Actually, the mixed up communication in this place is tragic.Don't expect regular updates! The nurses act really nice-- and some truly are-- but they just ignore/ don't hear/ too busy to answer my mom's yells. My mom couldn't use the call button. I had to stand outside and call in for help ..and many times none would answer. When they had a covid outbreak I got my Mom out as quick as I could-- sure enough when I brought her home a couple of days later she had to go to the ER. Turns out she came home with a bladder infection, pnumonia, extreme diaper rash, and then Covid!!! Thank God she survived all that and is back home. Do not trust this place!!! Especially if your loved one has special needs!

My mom was there 4 years. The staff knew my mom and my sister who spent the most time there with her. After mom passed not a one has called to extend condolences. It’s about the money that’s all that matters. Footnote: be sure to check medications for exp dates mom’s medicine had expired she had to wait for refill before she passed comfortably

We have a lot of family in the San Jose Area, so when my 89 year old uncle needed long term care, due to his dementia, this was the perfect place for him. Even with his limited faculties, the staff was able to engage him in many activities, including preparing food for the homeless!. These social pursuits are imperative to his maintaining a quality of life that he would never be able to experience on his own. We are so appreciative to everyone here. My cousins visit daily and are extremely satisfied with the care he is receiving. This place is welcoming, warm and wonderful.

The administration here is aloof and detached. The nursing staff are poorly trained and hardly communicative because of language barriers. During my short stay i was exposed to bacterial infections and i had an open picc line close to my heart. The port was left open overnight on more than one occasion. I was given a commode to use that had dried feces from other patients here. I was given rotten milk and lukewarm tap water to dri k. The response by staff for call button assistance was delayed and non existent at times. The building is dated run down and dirty. I would not recommend this facility to anyone.
