I enter the facility the atmosphere is friendly and inviting from the lady at front desk to the administrator who knows every resident to the staff down the hall working to ensure each resident is given the upmost attention. The residents look clean and the verbal residents verbalize that they are being taking care of. They are doing a great job taking care of our love ones!

I had my brother there for respite care due to the passing of my father. The facility itself was very nice and staff took care of my brother needs to there best abilities because he is total care but what bothered me the most he came fully dressed and plenty of his own clothes to wear that has his name on it but when it was time for him to come home 17 days later nothing was returned blankets lost, his special cup gone and medicine not returned. I called back to talk to someone and the young lady said the woman that handles that was gone for the day @ 11:30am so she replied I will have her get in touch with you. And still no response 😕

We brought our mother to this facility for rehab. After numerous clinical and dietary errors we are just DONE w them. COME TO FIND OUT, this facility was surveyed by the state and have so many issues from mistreating residents they might get shut down. Fingers crossed 🤞

My father in law lived there and was not cared for as he should have been. He had a stroke and had to be placed there. He had a g tube so he depended on the nursing staff to feed him. On his death certificate it stated he died of dehydration. So I guess that can give you an idea of what kind of facility it is.

This place is understaffed. Residents are neglected and left lying in stool and urine. Please don't come at night. 99% of the staff are sleep. Food is left in the refrigerator for a week at the time. Think twice before bringing your lived ones here.

My wife and I are very pleased with the care that the staff has provided her mother at the Rich Square rehabilitation center. On a scale of 1 to 10, we're giving it a 9. We would have given the rating of 10, had it not been for an elderly resident gentleman roaming the halls without his mask in plain view of everyone. All of us together, staff, residents and visitors need to work together to do everything we can to lower the risks COVID-19. In addition to receiving the vaccines and the boosters, we all need to be wearing our masks. This is a public health matter not a personal freedom issue. Omicron is more contagious and in all 50 states.

Rich Square health care nursing home is one of the worse ones I have ever been to. The lady in the business is so rude and ugly. She has a very ugly attitude with the people that comes and try to see their love ones. I don’t know who hired her but whoever did need to think about some things longer. I wouldn’t let my dog be in her presence. If you need a nursing home please let Rich Square Health Care been your very last choice.

It is a great place compared to other facilities I've seen. A few of the rooms need maintenance work on something's but nothing major. Just repairing cabinet door on night stand.
