Our good friend and father is left in pain from 12 hours to days at a time before anyone will even attempt to give him his pain meds. He is in agonizing pain and it's like they don't even listen to his calls. The doctor will not even speak with out family member/friend so they do not know the doctors name nor how to reach him. They also gave the excuse that there was no meds of his type in the facilities. This was a bold face lie. This place also does not clean dishware leaving our family member to have actual thick mold growing in their dishware. I'll stop there for now.

This place should not exist. Not only that, but the staff is so incredibly evil, cold hearted and cruel. I thought I was going to lose my mother and they treated me as if I was a nuisance because I would call to check in on my mom. As if they were so annoyed. They have caller id so they would chose when to answer my calls. I would go days without being able to contact someone. My mother left there in an emergency back to the hospital. She was malnourished, dehydrated, got a UTI and bed sores. My mother was there to be cared for, not abused. She was unable to change herself, feed herself or talk so she could not stick up for herself. I would show up and all she can express to me were her tears. My mother was a honored nurse for 50 years at San Antonia and for her to be treated the way she was should call for serious action. Station 2 is a horror house. And, as a grieving daughter, Susan would stand in the hall and stare at me with her arms crossed as I would say goodbye to the other poor residence that have fallen victim to this facility. GOD, PLEASE HELP THESE PEOPLE. I am so grateful to have got my mother out of there and saved her LIFE!

My sister was sent there for recovery from surgery and wound care. Her wound only got worse and she is now back in the hospital facing more surgery. Her medications were given irregularly or not at all, the antibiotics were not given as prescribed and the wound care was nonexsitant. She was not helped to take a sponge bath or to wash her hair. When she arrived in the late evening her wound was seeping and soaking her gown which she removed and rang for assistance for 2 hours with no response. She sat there in paper underwear and a sweater wrapped around her until her daughter showed up the next morning. There is little to no care given to the patients. How can they still be open and collecting money from insurance for services not provided.Really it's less than 1 star.

Horrible place !! my brother picked up my mother yesterday and now she is in hospital in the intensive care unit with a breathing tube down her throat due to neglect on this facility part!!! They kept telling us she is fine but she lost 50 pounds in 4 months of being at this facility!!! Every time we would go to see her or try to call her she was completely unresponsive due to the fact of either the wrong medication or too much medication that is still being investigated!!! And she also picked up lice at this facility and their solution was to shave her head!! Please do not put your family members here

This is the worst place to bring one of your family members ,,they don't respond when needed ,they don't have enough nurses on Shift.They don't even care about the patients . If you have your loved ones there you need to check on them every day to make sure they're giving them proper Care. I can't believe that this place has not got reported or clothes down by now my mother is there And we would like to take her somewhere else but we can't after we put her there..

My family member has been waiting for over 2 weeks to see an actual Doctor, the nursing staff are doing the best they can to treat his sudden illness. If this keeps up I will be calling 911.

Wow, staff do go above and beyond, despite the place looking like it is (under construction). My father only stayed there for about 2 weeks. Rehab department and nursing staff were very nice. The two gentlemen in the front office, Kenny and especially John (some people called him JV depending on what he is doing that day I guess- he wears scrubs one day, but wear a button up shirt and a tie for the most part) them two are just as wonderful as the other other staff. Will bring my father there again if he ever needs a nursing home again.

They have taken really good care of my Mother for almost a year. They call me immediately with any changes, have great recommendations, and I can see the comfort they bring my mom. Every nurse or CNA I've dealt with has been professional, caring, and honestly did their best to care for my loved one. As another reviewer mentioned, it looks "under construction" there are ceiling panels missing because they're sealing the roof against leaks. No big deal. Doesn't affect the care. I'm very happy with Park Avenue
