Horrible place. No communication between staff on top of being short staffed and a lot of neglect. I let them know many times to please leave my father’s soiled clothes in his laundry bag and I would keep track of it and wash it. I even labeled his clothes in case it did get misplaced he came back home with little to no clothes it was all missing the staff would keep saying they would look into it. A lot of people asking for help for hours while I would visit with no one in sight. On his last days there I came to visit one night saw the nurse and asked how my dad’s day No went she said good. My father was stressed his feet were swollen from sitting in a wheelchair all day and his eyes were bloodshot and he had bloodshot eyes with yellow crusty discharge. I cleaned his eyes and he was telling me his roommate passed away in bed. I spoke to the nurse again told her he was saying his roommate died she said yes in the afternoon and he was still in the room it was 8pm. They were waiting for his roommate to be picked up by 9pm my dad wanted to go to bed the manager said they didn’t have any other rooms available so he would have to stay put in the cafeteria that it would take about 2 more hours. The nurse came to talk to me and offered to put him back to bed in the room with the corpse that’s the only option they have another staff even felt uncomfortable with that and said I wouldn’t want to be in that room either when my father responded angrily. We had to leave because it was late and passed visitation hours. It was a stressful and heartbreaking night. I had to fill out a grievance form to complain and they didn’t have any so I had to write it on a sheet of paper and was told the director would call me the next day. Never heard from anyone.

Doesn't even deserve a 1 star!!! Most awful place!! They Don't have any care at all for the patient. Patient beating on each other and a smart mother director. Pt was they only good thing, but my dad ended up in hospital for phnemua and wasn't breathing was put on a breathing machine and a induced comma. So please , not this place.

When i say that this is one of the WORST places ever, I truly mean it i honestly don't understand how this place has not been reported and shut down. My father was here for one day. There's more that i will not go into detail and that will be handled by our lawyer. PLEASE do not send your loved ones here. The hospital social worker was the one that made the decision on her own to send my father here after we called & left her a voicemail letting her know that we did not want him moved here for his rehabilitation to get physical therapy after surgery. The nurses are not attentive, The facility Doctor that we spoke to was a complete idiot and didn't know any of my dads history or let alone the reason why he was there. I'm saddened by the amount of people that are left here and are not checked up on by there family, the Negligence by the whole facility is on another level.

I was there in 2020 and some staff was nice others were extremely rude to me. I was told by one nursing assistant she did not want to care for me because I was a negative person. First time she was assigned to me and already she thought she knew me. 👀 Yeah, no she based it on what she heard from other staff members. They don't like having a patient fight for their rights. I did that and took me almost a year to get my release. I am a dialysis patient who had at the time a catheter in my right should that was never to get wet. It was why I insisted on a sponge bath instead of a shower. Nope, there was one head nurse who said it was a shower only and I wouldn't have to worry about it getting wet they would keep it dry. NOT, I got an infection from them being unable to keep it dry in a shower. It got wet four times at least and I had to force them to release me home. I would not recommend anyone going there. There is always going to be good and bad in these places but this one violates your patience rights and no one has that right. I may had to place one star but it is a ZERO STAR.

I’m really pleased with the physical therapy and the speech therapy of Park Avenue the food. I was surprised that my guy that I am his caregiver he really he liked the food he it was tough for him because he had came back from a huge Alzheimer’s episode four weeks in the hospital From four-point to to free and happy thank you Park

My mom has been there all week and they treat her bad she don't get no conditioner after a shower or any lotion from them. When she asked they said she had to bey her own idk why they treat them like that. I would buy her some but why talk to her that way seriously. It's sad. I'm going to talk to a lawyer about the way they treat them.

My Mother was placed at Park Avenue Rehab on September 3,2024 She Fractured Her Left Hip and needed to get mobile again physical therapy they promised to take care of her We would come visit her almost every day she seems scare but my mom can not commit very welll call every week to see how her progress was on Never did they give me a call back. On September 17,3024 The nurse they where having trouble with her feeding tube so the took her to UMC Tucson where the said they fix Park ave sent my mom There time stating her feeding to was not working again So they took her to UMc Tucson ER and demanded she get a G-tube Place or They where not take her they left her in the ER. The UmC Tucson ER notice Serious signes of neglect she was dehydrated her legs where in traction and she couldn't not move them That mean They never did anything Physical Therapy on her at all so the reported they To adult protective services please don't ever have a Loved one stay there There is a lot more but I don't have alot of room to write very thing Down This really Happen to my

I’ve worked at several skilled nursing facilities and this is by far the best. What a difference great leadership and culture makes. I would highly recommend Park Ave as a place to work.
