I had a fall 9/26 on 9/27 they decided to do an impromptu discharge. They also refused to take me to my Dr. appointment. I woke with chest pain and shortness of breath; the nurse gave me a breathing treatment and said the Dr. will be in to assess me and order a chest xray. That changed because they told Dr. Erik Maskin they wanted me discharge. It was Dr. Maskin and Megan Ponder who approached me. Told me I was medically cleared snd cleared by therapy; although on Wednesday at my care plan meeting they said I had another week. The Dr. also charted on 0/36 I had another week. I called 911 to go to ER they tried to send AMA papers with me. Fast forward after 6pm that evening I went to nurse station because I have not received my dinner yet. At they time I saw the nurse supposedly assessed me after fall; I protested that she didn’t do a body assessment on me and it was approx two hours after fall a nurse came to see me. Another nurse decided to antagonize me; at which time a lady in 122 was on floor. As a nurse for which I am I proceeded to tell them the order of operations things should happen with the incident. They didn’t like that to well. The four nurses went in the admission nurse office and came up with a story. Scottsdale police arrived did not get my side of story. They yanked me out my wheelchair and continued with excessive force. Fast forward I was taken to Honor Healthcare where I currently am from 9/27. I was said to have pneumonia; elevated troponin over 300 had a cardiac the following day. The hospital wanted to discharge me to another SNF when I’m medically cleared. I was accepted at Mission Palms until Osborn called to tell them don’t accept me. Megan Ponder, Asya Currie, Martezia, DeDe Darnell, Darna De Guzman, Frfrivk Abuan, and Gabriella Thomas all falsified documents under tge direction of Courtnee Scrivener; reportable to State Board of Nursing and legal action for negligence and defamation

I am a 74 year old Cirrhosis Patient with complicated issues and multiple needs. Osborn Rehab was the second such facility I had entered in a year and I was reticent to go based upon my prior experience. After my friend and MPOA did research on multiple facilities in the Valley, he received a call from Jason. That conversation was the deciding factor and from there it was a great experience as far as hospitalization can be great. Some staff were better than others, but none were unhelpful, impolite or unwilling to help. Room calls were answered and my guests were treated politely. Rehab and hospital food is OK at best and Osborn is no different. Some days better than others, but that is not why you come here. I hope this review helps someone decide.

My papa fractured his hip and was very weak and a terrible experience at another place. The therapy and nurses at Osborn did a wonderful job with the whole process. We are so happy for him to be in such a clean well run environment ! He made such great progress.

My family and I had exceptional service and care with Osborn in Scottsdale. My grandfather was taken care of every step of the way. The nurses and staff were outstanding, from being friendly and thorough they provided the best care. They truly put his needs in the forefront. Thank you Osborne for being the best for my loved one.

Osborn Health and Rehabilitation has been like a 2nd home to me. When I first left the hospital knowing that I was going to Osborn Health I was very skeptical because I have never been a rehabilitation facility and I didn’t know what life would be like in there. It was the best decision me and my family made. I lived and recovered at Osborn Health for about 3 months and by the time I left the facility everyone there was like family. I can’t say enough about the care and service I got from the staff at Osborn Health. From the gentlemen who run the facility, to the nurses and CNA’s, reception, kitchen staff, hospitality, even the cleaning personnel, I was on a first name basis with all of them and they are all so kind, courteous, caring, and professional. If I’m ever in a situation again where I’m required to have post operative care, I would choose Osborn Health again in a heartbeat. Thank you to everyone there you guys are the best!

Please, if you want your loved one to get better do not bring them to this facility. They did not care about my family member, frequently left them unattended when they truly needed assistance, and the staff is not proactive. They want to extend their patients stay as much as possible it seems like, probably for financial gain. Please look elsewhere as my family member wasted precious recovery time here!! Half of the 5 star reviews you see are from employees and not truthful. Stay far away!!!

If I could rate this place a negative 100, I would!! Unfortunately, my brother passed away in this facility. He stated that he was neglected, not being helped right away when needed. Wait time was at least an hour. Unfortunately, the nurse didn't give much information on how he passed nor did the police. Listen to your loved ones ❤️

This place is HORRIBLE! My love one was admitted her for therapy, which she really loved and enjoyed! She was promised a lot of things she never received. The care was disgusting, her call light was unanswered for 30-45 minutes, the nurses administered ALL her meds at once without waiting until she had taken them and EVERYONE there falsify documentation ( her medical records are a legal document). Nurses falsify documentation without even accessing her, listening to her bowels or ask her questions about her well being. She had a fall after a CNA emptied her foley bag and didn’t clamp the tube, so urine was all over the floor. She had her call light on, no one answered so she tried transferring herself from the bed to her wheelchair and slipped on urine hitting her back and head. I received a call from her telling me what occurred, I sent s text to Jason, Admission, Courtney, DON and the Social Worker Assistant informing them of the fall and letting them know the neglect was unacceptable. On Friday, September 27th the Social Worker called informing me that she was being discharged. I asked how when I attended a Care Plan meeting on Wednesday, September 25th that she had one more week. They decided that morning that they no longer wanted her in the building and refused to transport her to a doctor’s appointment.There was other occurrence leading up to this neglect. The facility ended up calling the police on my love one saying she was threatening staff and was trespassing! How was she trespassing when she was a patient. The Scottsdale police department came, threw her to the floor and put her in a 4 point restraint. My love one was there for therapy due to a Stroke, she couldn’t walk alone without assistance and balance was unsteady. Not understanding why both Osborn and Scottsdale would use that type of excessive force
