North Bay Post Acute

The facility is clean enough, and the patients staying there have freedom of movement, but my mom was discharged with no notice, and before she was ready to live at home alone. I was called on the morning of her discharge, saying she was ready to go home that day. I live 4hrs drive away from the facility, so I had to take the rest of the day off of work, drive down, get my mom, and drive back, meaning I had to take the next day off of work, too. When mom was discharged, it wasn't discussed with the social workers at the facility, so the paperwork that was needed wasn't quite ready for me when I got there. Mom was supposed to stay until she could take care of herself. They determined that she could do so because she was mechanically able to feed herself from food that was prepared for her, and she could somewhat get back and forth to the bathroom. The staff that I interacted with were all pleasant, but the facility's desire to push mom out before she was ready was unforgivable.

My sister was in the facility for rehabilitation from a broken leg. The administration was very helpful in all aspects of her stay, especially her rehabilitation and coordination of return to home. I was able to visit just once and what I observed was an overall general concern for patients, an old but clean facility, and a staff who cared. LC

Friend there several times.Majority of rooms are not “private” but double or triple bed rooms,bathrooms are shared between rooms. Facility is old but seems clean. Often short staffed , staff are very nice as a rule, some go out of their way to please patients.Often seems disorganized with not the greatest communication among staff. Regular social worker on medical leave.OT,PT and speech therapy good. Hard to get ahold of anyone to talk to in the evening or night. Phone goes unanswered. TVs often don’t work, but do get fixed quickly. No phones in rooms, most patients have cell phones. Food has been OK. BUT…. Kitchen was suddenly shut down and as of yesterday the patients had been getting sandwiches. Salads(cold foods) and no hot drinks since Wednesday 4/3/24 and as of yesterday still no reason as to what the problem is, when it will be fixed and when the patients will get a hot meal. Concerned re lack of transparency, For these issues I would not recommend at this time. Now paper is printing what many had figured…rodent issues in kitchen. A week out kitchen still closed….

My niece has been here over a month. Nov 5 she complained of an ingrown toe nail. Shes now hospitalized with bone .infection from toe nail.

These people are the ABSOLUTE WORST !! the entire staff has several screws loose. 2 of them that specifically need to be fired. In order .. Danica she's super slow in answering the call button and gemeni doesn't seem to know what heyte doing. Nobody at night time, broke tvs TOTAL LACK OF COMMUNICATION (which is paramount for a place Luke this) didn't give me discharge papers and instructions. I need insulin and the other meds they were giving me first at santa rosa memorial which SUCKS JUST AS BAD. whole staff needs retraining or let go. We go ignored and NOBODY answers the phone . They give me the wrong foods and drinks (I'm type2 diabetic) People's lives depend on their help but it's nowhere to be found.even the PT people are stupid. A person could lose their life there and thr patiens that I saw there during my 20hour stay whi h I cut short, all of them agree to what I'm saying and followed with " Big Time " I wouldn't let my dead dog go there. I plan to go public with this and if anyone wants join me as I stick it to them please feel free and get your own justice. I STRONGLY ADVISE ANYONE GOING TO SANTA ROSA MEMORIAL 'ESPECIALLY THETRAUMA CENTER . they're also inept and all their answers I asked were based on their opinion ONLY and not by fact. These people here should never be opened. I wish I could put a minus 100 for their ratings. Don't trust them for a minute cause you'll regret it.im very angry but I'm focusing on what needs to be said . I donr lie, I don't exaggerate for simple reasons of being caught. I'm straight forward and honest as they com. This place is BELOW the bottom of the barrell .

I cannot say enough about the exceptional care my grandma received. Every person I dealt with from the person who cleans the rooms to the CNA‘s to the nurses and to the administration, treated me with the upmost respect and were dedicated to my grandmothers well-being, and to the recovery of my grandma. Their attitudes were always cheerful, and they all went out of their way to make her stay comfortable. They were always there day or night to help my grandmother.

What a poorly operated facility. The staff that I interacted with had zero empathy. Emergency or call buttons do not turn on the red light in hallway above some room doors. They said they would fix them 3 weeks ago. No solution given to patients for the meantime. They literally have to yell out towards the entrance of the room. Rounds to come check on patients happens maybe every 5 hours, maybe. Moral is low, they seem to be way understaffed. Just a very sad situation for the patients.

If I could give this place and the staff from administration down to the CNA's a negative # of stars I would. This facility is terribly under understaffed. The staff allows the patients call lights go unanswered for up to an hour, sometimes longer. I personally observed patients yelling "please help me someone help". For over twenty minutes or more. I've heard staff members yelling at patients. Don't get me wrong there were a few good employees but no more then handful of employees that were good and you could see that they weren't just there for a paycheck, they actually cared and treated the patients with respect. I even had an charge nurse (RN) give me another patients medication to use on mom who was a patient there. In closing I just wanted to say "I wouldn't take a dead dog there for any kind of care. It is a GOD AWFUL PLACE
