DO NOT LET YOUR LOVED ONE COME HERE! Maple city has a representative at hospitals very far away looking for patients. If your facility was such a great place it would be filled with local residents instead of finding residents to ship out and take them away from their families. We all know the system is full so the hospital will make you take the first open bed. I know many residents that their family has to travel far to now come see them. This place is the most unorganized facility I have ever seen. Pass the Buck is present every day. The smell is absolutely horrid. Don’t let them put your loved one upstairs it’s a nightmare I don’t ever want to relive. One nurse to hand out meds to the entire floor. Residents our sitting in their own filth. It’s not even feasible to get a wheel chair in the bathroom. Four patients share one bathroom that is not wheelchair accessible . Now how is that even aloud. If you have a loved one currently there and our trying to get them out DO IT YOURSELF. They will not help you. Call you back or give you a straight answer. They have a social worker but she did nothing to help us. My loved one got sick and I took them straight to the hospital back home. I dropped everything and signed them out and drove them back to a hospital in my hometown. The hospital had to then help me find a place closer to home. I did all the leg work myself. The system is messed up and no one will go the extra mile for you. You will have to put in the work calling nursing homes every week following up and doing tours!! It is a lot of hard work but in the end to not have your love one at Maplecity will make you sleep at night. To be able to walk into a facility that actually smells like a normal house,clean, engagement , activities for the residents. We were blessed to find a place like this but it took a lot of work. Your loved one is worth it. Please do whatever it takes to keep them from this place. Maple city I don’t need a response either you never responded when we first told you what we needed so don’t attach a bull crap I’m sorry to hear of your experience. Maybe look at how your facility is run and question if you would let your parent live out their last days in a nightmare! I hope and pray that every family finds a way out!

The care our mom got while in this facility was absolutely amazing. While yes they are short handed like every where they gave her amazing care! We lost her yesterday and I will say at least 25 staff members from anywhere in the facility from admin, CNA, LPN, RN to house cleaning staff came in to say their goodbyes and most had tears! Thank you for all they did!!

Very run down, seriously neglected residents, very poor infection control. This place needs to get shut down and bull dozed. Primary nursing staff is agency and much of everything is disorganized. Shameful.

Horrible place. The smell of urine hits you as soon as you enter, residents in doorways asking for help, dirty linen piled in corners. Keep your loved one away from this place.

Our 95 yr old mother was there for about 6 months. Received great care, the nurses and staff really treated her well and truly cared about her. She received therapy when needed, and at her end of life, they did all they could to keep her comfortable. Great communication from the nurses and all involved. We were there frequently and always felt welcomed. This place is definitely better under the new management.

Worst place Ever lack of employees nurses and regular staff. Patients calling for their meds only 1 nurse to hand it out so my step dad has to suffer in pain. Cause of the lack of employees you could be in your soiled clothes all day. I’m not writing this cause I dislike anyone. I do this cause it’s the truth soon he will be getting transferred not a good place to put a loved one if we could have care for him at home that’s where he would be

Trying to get my Dad transferred to Wellsville and social worker was less than nice. The Manor messaged her for the info to transfer him on Tuesday . I was told Wednesday she didn't get anything, then oh yes here it is. Now being told they can't approve him for transfer till monday. Should it take this long just to change facilities. That's why we have computers.

Staff are fantastic and friendly and work hard to make sure the best care is provided for residents. Rehab team is awesome!!
