Macclenny Nursing and Rehab Center

I want to express my deep gratitude to the entire team at Macclenny Nursing and Rehab for the incredible care provided to my mom during her short-term rehab stay. The level of compassion and attention she received was outstanding, and I couldn’t be more thankful for the professionalism of the staff. A special shout out goes to Cody, who truly went above and beyond to make our experience exceptional. From the moment of her transfer to the facility, throughout her stay, and even ensuring a smooth transition back home, Cody was there every step of the way. His dedication and attention to detail made a world of difference, and we are deeply appreciative of his efforts. Thank you all for the exceptional care you provided. You made a challenging time so much easier for my family, and we are forever grateful.

My brother admitted here a couple months ago in very poor shape. The nursing and therapy care have helped him progress much more than we originally expected and he is doing great. The staff and leadership are very responsive and caring.

My mother with dementia just spent 5 days for respite care … She actually enjoyed her stay and we look forward to using the service again very soon … The nurses were great with her and allowed her to sit at the nurses station and she thought she was working ! I cannot wait until our next visit !

My parents were discharged from Macclenny Nursing and Rehab today. They were there for about 6 weeks. In that time the amazing PT and OT department took a bed ridden patient and diligently worked with her until she is able to walk with her walker. My dad has severe neuropathy which made him unstable on his feet. Bruce is wonderful. He worked motivating my dad building his strength and stamina. I can’t say enough good about Kari, Jennifer, Taylor, Bruce and the whole crew in the gym for genuinely caring about my parents and helping them achieve their goals. Speaking of caring, that describes the nursing staff and CNA’s to a tee. I can’t say enough good about the efficiency of the nurses and their general concern for their patients. The CNA staff is patient and responsive to their patients. It would have been much easier to change my mom’s brief, but she wanted to go to the restroom. For much of her stay, moving Mom was a two person job, but they did it allowing her to continue being continent. I can honestly say we did not get this level of support and care in the other nursing facility we were in. I am grateful for the people at Macclenny Nursing and Rehab from the top down and the bottom up. EVERYONE there cares and does their best to make sure their patients are in a safe, clean environment, respected and cared for. The management is responsive and works hard to ensure needs are met. We were there for rehabilitation, and I would not hesitate to go there again. Thank you to everyone who helped us.

Staff is the best ever! awesome! They care! I'm still learning about how everything operates and so impressed with the knowledge that they have about their patients, from front desk, managers, nurses and all staff! And answering all my questions! They all know my mom which shows me they care and the care provided is genuine! They care about their patients! Thank you so much, it is so assuring to know , that times that I cant make it, their "all" involved in her care.

Excellent care We could not have cared for our mother as well as the nurses and staff at Macclenny Nursing Home did. The physical therapists went above and beyond as well. Would recommend anyone needing care or rehab to check them out

My mom and dad were residents here over 8+ years ago and then my dad passed away in 2014 and my mom had Alzheimer’s and this nursing home decided they could not care for my mom any more so they called and found another nursing home to send her to and wanted me to move her, I told them that I was not able to move her and that they needed to get a medical transport vehicle to move her. After my mom made it to the new facility the administrator of the new location called me and told me that Macclenny Nursing and Rehab had put my mom in a taxi cab and sent her to the new facility. My mom having Alzheimer’s didn’t know or understand what was going on and she was non verbal and the taxi cab driver had told the new facility that my mom was hitting him and tried to bite him while he was trying to drive her to the new place. It is some sorry people who would stoop that low to have a patient that was non verbal and had full blown Alzheimer’s transported by taxi cab. DO NOT USE THIS PLACE IF YOU TRULY LOVE YOUR FAMILY MEMBER!!!

Rebab staff are excellent, especially Bruce and Erica. All staff were friendly. The APN was especially helpful. Would definitely recommend this facility for rehab care.
