I had a very good expperance with my stay at Legacy Rehab and Health. The nursing staff seen to my every need. The lady’s that cleaned my room kept it clean and were very courteous. I cannot say enough about how thrilled I was with the love and care that my brother, Frank, received from Legacy Health and Rehab – Pleasant Grove. I moved Frank to Birmingham after his dementia diagnosis just about the time the world shut down with COVID. As his health declined he was moved once before landing at Legacy. I was impressed by the low staff turnover; I saw the same faces month after month. I was equally happy that he was always clean and well cared for. But more than that, I was thrilled by the way he responded to the staff of Pleasantville when I was there visiting. It was obvious to me that the care and attention he received while I was present was matched by the love and care he received while I was not there. His reaction to the staff could not be faked or put on for my benefit. They cared about and loved him, and that meant so much to me. If I had to do it all over again, I would have moved him to Legacy earlier. He obviously got better care there than either of the two other facilities he stayed at in Birmingham. Thank you, Legacy, you are the best!!!

Legacy has taken very good care of my sister. It took her a little while to get adjusted. But now that she's adjusted she enjoys her day. She loves the activities they plan for the residents. The new administrator is very nice, professional and seems like he care for the patients. The other staff members are very caring and loving toward the residents.

They will not let you speak with your loved ones via telephone. They are hiding something within the business itself. I have called for a welfare check of a loved one and the Pleasant Grove community police department never visited the center; they said they'd call me back if there was a problem; they called back within 5 minutes to say I could get through; I'm still calling non-stop to this day and no one will answer. This place is a total sham and should be shut down; there is no customer support for on behalf of your loved ones there!!! They are just profiting from your family that needs actual help without actually helping them at all!!! This place is just another to put human beings that you want to have die by the system. This place needs to be abolished once a for good for the sake of all humanity!!!!!

My mom was there and we lost her Wednesday. My grandfather went to get her stuff. But just so happen her brand new phone was missing. As a nurse walks buy and here's him say he's filing a police report she says hold on and come back with it. My grandfather tore that room apart looking for it. so if u want your loved ones stuff stolen and have people lie to your face about it when you been through enough. There's no way in hell I would send someone there if they are going to do that there is no telling what they do. That nurse should have lost her job and should not be there anymore and the manager should be investigated.and don't try to call because they won't answer. They should be closed down. And by all the reviews already I don't know how they are still open. But I do know it will come back to them what goes around comes around.

My mom is currently at Legacy Health and Rehabilitation of Pleasant Grove. After a week in the hospital, she was referred for rehab services for an additional time to recover due to experienced trauma. She needs therapy to help her get stronger and regain the physical abilities she lost before coming back home. I thought the transition from hospital to rehab would be difficult and disorienting for my mom but just the opposite, we barely waited to be helped when she arrived to check in. The staff were all very friendly and helpful. I especially loved that those who would be overseeing her care introduced themselves and really took time to make her comfortable. From the moment she entered everyone has been attentive to her needs, open and point on with her plan of care. They have graciously involved my mom and I in conversations relative to improving her health, physical wellbeing and care. After one-week, administrative staff set up a meeting with my mom and I with the Nursing Team, Physical Therapists, Floor Unit Management, MDS Coordinator RN’s, Social Services, Admissions, Restorative Nurse, Finance, Housekeeping, Dietary, Activities Team all of whom provided updates on mom’s current plan of treatment and care. We also collectively discussed options and how to prepare for the next steps after she is discharged. As her at home caregiver, I appreciate the Care Team for taking time to go over and clearly explain their assessment of my mom’s condition and treatment options….even more the patient and helpful instruction with her insurance. It was obvious from the tone of all those that attended the meeting their genuine concern that her discharge from a managed care facility be coordinated, smooth and safe. Caring for others is a big responsibility. Legacy Rehab is more than a skilled nursing facility, they have become family. I am so glad I chose Legacy Health and Rehabilitation of Pleasant Grove, LLC and would highly recommend it to anyone in need of short-term rehab services in an inpatient setting. OH…..MOM raves about the FOOD!!!! HAT’S OFF TO DIETARY!!!!!

First and only night there the bed was like a rock. I had wound care patches on my lower back with very thin skin. I lay awake that night crying from the pain. They gave me Tylenol. Dinner was half a cup of blackeye peas, okra and mac and cheese. Breakfast was a couple eggs and grits. The food tasted old too. I went there after a week in the hospital for rehab for a week and couldn't do it. I was so weak .
