The good things: My mom recently spent about a month there after breaking her hip in a fall. From the time she entered, until now, she has improved immensely. Their food is excellent, healthy and nicely prepared. The bad things: they give each patient a red button that is attached to bed. Obviously, you ring it if you need assistance. It's a joke, they take between 1-2 hours to respond. They don't even walk by your room to see if you are okay. The care is inconsistent, depending what CNA you get. The nurses are different at least three times a week. The communication between employees is almost non-existent, nothing is related from shift to shift. The male receptionist is great, the female doesn't even look up from her phone when you walk in. Billing department is excellent. Sometimes it took days to catch someone in their office, then they say they have to much to do. There is a general manager who is "there", but not involved where the real problems are. They need an operations manager, not a manager who is in his office all-day with no clue what is going on in hospital. The van driver was awesome! So there are good parts, and bad parts. My advice, keep a close eye on your loved one while there.

I'm beyond disgusted with the treatment I have received in regards to a girlfriend of mine that was just discharged from there. I am after doing some paper reading and some investigating of my own dumbfounded at the fact that she was put in there because of hypertensive heart disease and in actuality the problem was she has a slipped hardware in a knee that she had surgery on years ago and it's keeping her from being able to walk while being there she hurt her shoulder exercising on top of the knee problem when she needed surgery on her knee first not physical therapy she hurt her arm while doing exercises so now her right knee and her left shoulder is out I can't barely move her she's having a hard time and I've called various times throughout the week to ask different questions I always have to leave a message and I always get no call back it's very disheartening when you're doing the best you can to care for somebody and you can't even get them to return a phone call thank you Sarah I waited all day to hear back from you and nothing called three times I spoke to you in person once and was told you to get back to me at 10:30 or 11:00 it's now 3:00 and I've called three times having to leave a message that is blatant disregard for somebody I have to care for and I don't appreciate it at all. Seems maybe I will start writing letters and emails and see if that helps. It's imperative that they send these patients home with the right amount of medication that they need until they see a PCP which in fact is the scheduling that they did at the facility and she's almost out of her medication and we don't get to see her PCP for two more weeks I just don't even know what to do I'm disgusted.

My mom was there and the PT department was excellent. Medical and nursing care very poor and unprofessional. Nursing made a major mistake that caused her to have to stay longer. Food was absolutely horrible. I do not recommend this facility.

This is a joke of a healthcare facility. My grandma was there for less than 12 hours before we pulled her out. This place needs to be shut down or have a whole new staff hired because they are heartless and unprofessional.

I am so pleased we chose lakeport post acute!! I couldn't be happier in the way they are taking care of my brother!!! Very family friendly, clean facility, caring, and professional.

Our dad couldn't have received more quality, kinder, compassionate care. We are grateful to the entire staff at Evergreen. Our wish would have been to have dad home, but this turned out to be the best option. Thank you Evergreen. Like other families, it takes your breath away when you have to make the choice to bring them to a facility. But when you take the time to watch the staff at Evergreen, and their interactions with the patients, your heart is comforted greatly.
