Lake Worth Rehabilitation Center

Six months ago my dad had a terrible fall and was in and out of 5 hospitals and 4 rehab centers. It truly is a miracle that my dad was able to come home in part largely due to the care he received at Lakeworth Rehabilitation Center/ Elevate Care. From the friendly reception staff, nurses, nurse assistants, PT/OT staff and maintenance my father received excellent care. The first day my sister and I walked in to visit the property we saw 6 PT/OT staff members working with residents. A huge thank you to Rosemarie and Marjorie the charge nurses who were on top of my dad’s care and addressed all his needs in a timely manner, making all his appointments and communicating with his doctors. Pearl, Yanique and Laudy were the nurses who went above and beyond for my dad’s recovery. Gilberte, what would we have done without you; World’s best nurse assistant. The miracle workers: Jose, Wina, Christine, Owen, Dede and Claudia the therapy team. My dad arrived unable to prop himself up in bed and after 3 months he is walking 32 steps today and doing all of his exercises. I would also like to thank Maddie for all her organization skills and follow through from day one admittance to discharge. Again, we are so grateful for all of you!

Worst place ever for patients who are unable to move and seeking rehabilitation. My mother has a bed sore. She received little to no physical or speech therapy. My sister and I spoke to several staff members for supervisor to administrative. They have little to no compassion, Jemely, who is DON, and the nurse supervisor Rosemarie are the worst. My mother was slowly declining during her time there. She was rushed to the emergency room because her respiratory was decreasing and found out she had pneumonia. From there, it was a perfect opportunity to bring my mom elsewhere for therapy. If she was to go back to that place, they would have surely and slowly killed her. I wish there was a zero star as an option.

A dear friend of mine has been at elevate for a few months. I have never seen her in such good shape, she’s cheerful, she is well cared for the staff is friendly and helpful. When I go there it really seems like a place that is comfortable and Organized and keeps the idea of their residence well-being in the forefront. I would definitely recommend this place if you have someone that needs that kind of care.

Truly excellent. I recently moved my dad here and the service exceeded my expectations! This is the cleanest facility I’ve ever been in! My dad is so much happier here! The staff is phenomenal. Thank you to Kevin in admissions, Nora at the front desk, Paula the DON is the BEST, John the ADON is so friendly, and all the nurses and staff that care for my dad. Thank you Marjorie for taking care of my dad in the evening. It’s so nice to have my dad somewhere safe and where people actually are happy, actually care, friendly, and always willing to help! Huge bonus is they always answer the phone and have incredible communication! Thank you. Elevate!

My 92 year old Mom gets the best care. Everyone is wonderful. They really care about the patients and their families. Mom is always clean. The facility is always clean. She really likes the food & you can tell a lot of thought and care goes into preparing and serving it. Everyone is very caring and available if you have any concerns. My Mom has been there for a year. Having a loved one in long term assisted living is hard. I find comfort in knowing I picked the right one.

Clean and friendly place. The place is very welcoming, you are greeted from entrance and staff are very helpful and accessible. It is easy to get answers if you have questions and whatever you need the staff gets it done right away. The residents were on the dining room and the set up looks very pretty and the staff are supervising and very attentive to the needs of residents. Staff looks like they are happy to work here.

Every exchange with staff from entry to exit has been pleasant. I met Maddie the SW and felt immediately my friend was among people whose heart and soul were in the right place. Impeccably clean facility.

I'm so happy and appreciative with the care my mom has received at Oasis. She has been here for almost two years now. Everyone that works here is so friendly, helpful, and knowledgeable. They always make me feel so welcomed and at home. The facility is always very clean and well maintained. The food is really good and tasty. They have a wide selection of menu options to choose from each day. Oasis has many fun daily activities for the residents to do. There is a beautiful courtyard that has a really nice gazebo, and a section with tables and chairs. If I ever needed a short or long term facility, I would choose Oasis for myself. In my opinion, they deserve 5 stars! ★ ★ ★ ★ ★
