The change of ownership has been great for the facility. Their annual state inspection was great. The managers are willing to listen to your feedback and/or concerns. They also have a fantastic pet therapy program. This picture is of Monty. He is the purrfect pet therapy kitty! đť

This place is horrible the staff doesnât care about your loved ones, my husband was there 2 weeks they sent him to the hospital without notifying me, and had the audacity to tell me they donât know what hospital he was at and I had to call to find what hospital claiming he was in lake city actually he was in Gainesville hospital. Then Tarshu the director a want to be educated lady pointed her finger in my face, then another black lady name Regina floor unit nurse came out a room like they wanted to fight me. My husband doctors appointments was never scheduled to start his MS treatment then on top of it they lost his folder.. of and the front desk girl Tori sat there and lied that Tarshu didnât point her finger in my face.. oh boy I will be calling the State..

My sister in law had to stay in this facility for 5 weeks. Everyone was extremely helpful with the process and made the experience easier. The facility was clean and no matter what time of day I came the staff was helpful and polite. A special thank you to Renee and the book keeper. They were both patient and made sure my sister in law transition home went smooth.

This is a wonderful place ...I am so so happy with this place they take great care of my hubby the food is amazing here and they give the residents plenty. The staff is amazing. My hubby is here for rehab because of a stroke and I will say the rehab team is great.

Do NOT send your loved ones here. This place packs bodies in like sardines and thatâs exactly how they feel about your loved ones they are just random bodies. Staffing is rude, the food is disgusting, will refuse you to go get medical treatment and the place is filthy. My loved one was to get physical therapy and received minimal therapy, was left laying in a bed all day and then told to leave before could even walk fully due to insurance issues. They do not communicate with point contact, all they do is stress out the patient by taking issues to patient verse patients contact that deal with insurance issues and finances.

My mother stayed for 3 weeks. I want to say that the CNA Brandy was so sweet and caring. She made my mother and myself feel loved and cared for. We need more CNAs like her. PT Kimberly was so sweet as well. She would encourage mom and push her so she could come home. I appreciate all the support. Matt, Lisa, Sandy, Tarsha, Renee, Quinesha ( hope I spelled that right) and the sweet older lady that works the front door. BRANDY you are the best!! I am going to keep I'm touch. â¤ď¸

These people don't care about the residents here. Especially the dietary needs. Constantly not giving you "Doctor ordered portions. Then talk down to the residents like they are an inconvenience to their routine. They don't realize they !re here for the residents not the other way around. If you have a choice don't send your loved ones to this place. It's more of a prison than a care facility. Believe me, I'm a resident here.

I worked in Lake City at Lake Shore Hospital ER for close to 30 years. I know first hand the horrible care that the elderly received back in the 90âs when it was Tanglewood and even when the name was changed to Avalon. Never Ever would someone I loved be admitted there!!! But I gave it 5 stars for several reasons, I am usually a tough grader, or so my lab students always told me, when I was a college instructor. But they truly deserve 5 stars, itâs very hard to overcome a bad reputation, and though itâs taken a while, they are hands down THE best around, thatâs worth 2 stars.My Mom was admitted in late January, it was NOT where we wanted her to go but they had a bed and it was a wait list for the other 2 places and we thought letâs take it and then transfer Mom when a bed opened up. When my sister and I went in to sign the admission paperwork, I looked for something, anything really, to voice my concerns, there was nothing! The admission lady was professional, in both dress and in explaining what each form was. It was so hard to sign the DNR, but she said you are a good daughter, donât ever let anyone tell you different, it takes a lot of love to put your Momâs wishes above your own, Star #3! The nurse taking care of Mom came in with forms for vaccinations, she explained them all, and the importance of them, she knew her stuff and thanked us for allowing her to be part of Momâs care, Star #4. Met the Physical Therapist and Occupational Therapist, both were friendly and professional, star #5. So there goes all my stars but I have to give Jodie and the nursing staff â¤ď¸â¤ď¸â¤ď¸ Triple hearts, they are truly deserving, they called daily to update us and notified us, on any changes, regardless of how trivial it may have been, as well as taking the time to answer any questions and NEVER seem to be in a hurry, that meant A LOT!! We pray Mom is home for good, but if it ever becomes necessary to place Mom back into a rehabilitation center, Avalon will be our first choice and not our last!
