**Concerns Regarding Facility Conditions and Staffing** **Facility Environment:** The overall condition of the facility is alarming. The food provided is often inadequate and does not match the menu descriptions. Numerous health and hygiene issues are present, including pests such as roaches, spiders, rats, and bed bugs, as well as mold in many areas. The unpleasant odors in the hallways are overwhelming, and litter, including cigarette butts, is prevalent. Furthermore, due to neglect, some residents have developed tunneling wounds, and others have suffered from maggot infestations in untreated wounds. **Admission and Staffing:** The quality of staffing and admission processes appears to be inconsistent, with a roughly equal mix of competent and subpar personnel. Those who do not engage in favoritism or gossip often find themselves at a disadvantage. Concerns about abuse of power may lead to retaliation, where individuals face repercussions for reporting misconduct. When state inspections occur, staff members without the appropriate CNA licenses are sent home, which highlights the importance of proper licensing. Unfortunately, reports of mistreatment towards residents often go unaddressed, allowing abusive staff to remain employed. Additionally, staffing issues result in inadequate care for residents. When staff members are required to fill in as CNAs, they frequently neglect their responsibilities, leading to residents being malnourished and dehydrated. Staffing management often manipulates schedules, leaving employees without proper notice and subjecting them to unfair treatment, all while offering insufficient compensation for the demanding work expected of them. **Nursing Staff:** The nursing staff also reflects a mixed quality of care. While some nurses demonstrate a genuine commitment to their roles, others appear to prioritize their salaries over patient care. Instances of medication mishandling and even narcotics theft have been reported, yet these issues are often swept under the rug to protect licenses rather than addressed appropriately. Additionally, nurses often face challenges in obtaining necessary supplies for residents, further compromising their ability to provide adequate care. **Certified Nursing Assistants (CNAs):** The performance of CNAs is concerning, with a significant number failing to meet the required standards of care. Many CNAs are difficult to locate when assistance is needed, and some may avoid responsibilities altogether. This neglect has led to serious consequences, including increased risks of aspiration and choking among residents. The expectation for CNAs to provide nightly showers in an understaffed environment adds to the unrealistic demands placed on them. **Conclusion:** The overall environment of this facility raises serious concerns about the quality of care provided to residents. Despite any positive reviews that may exist, it is crucial to recognize that many of these may be influenced by pressure from staff and administration. It is advisable for families to seek alternative facilities that genuinely prioritize the well-being of their loved ones, as the superficial improvements may not mask the underlying issues present within this establishment.

Messed up my elderly mothers most important Doctors appointment she will probably have in her remaining years of her life, and the one nurse that wears the dress everyday to work has got to be the Devil , I don't see how anybody makes it out of there Alive , and from my understanding from a good many Doctors they actually don't

My 88 year old dad has been here for 2 weeks on a short term stay. He has been here before and the care he received then made the decision to come back an easy one. The staff is wonderful. Very helpful, friendly and good to family as well as patient. The food is delicious and plentiful. There is always someone smiling as they work, one in particular Marquita Holliday. Haven't seen her when she didn't have a smile. The only complaint possible is trouble getting clothes from laundry. Even with the name on them they have a tendency to get lost. Overall a wonderful rehab.

I was apprehensive at first, since I had not that pleasant experiences in the Jacksonville. However, these misgivings were soon dissipated with the care given at this facility. I wish to tell everyone how courteous, professional, and warm each and every staff member I have encountered here. From Wound Care, to Certified Nursing Assistance , to every Nurse, to Social Services, and even House Keeping and Laundry. All act in highest standards of performance. This is no small act, as the staff numbers are sometimes lacking. However, they get it done the most with reduced numbers. A very fine example, indeed!

I was there at that facility almost a year ago, at first I didn't want to be there because the bed that I was sleeping in was uncomfortable but they brought me a different bed and it was more comfortable. The CNAs and nurses were all great , and the therapists were amazing. I give this place a thumbs up 👍

This is a wonderful place. All of the residents that I speak to on a weekly and sometime daily basis, always have good things to say about it. The employees that work there are great people,some have been there for a very long time and know all the patients very well. If my mother needed to be in a facility like a nursing home, I would not hesitate to look into this one. If this review helped you, please hit the thumbs up. Thank you.

I cannot say enough good about the care that my husband has received while at JH & R. The nurses, CNAs and rehab staff are amazing. They all are very caring, kind, competent and want what is best for the patients in their care. The admission process was easy. Taylor was absolutely the best in finding out information from our insurance about benefits and having everything prepared for his admission. Everyone has treated us like family. He has been in the rehab side of the facility and I highly recommend JH & R to everyone needing rehab care.

This place is disgusting bed bugs, spiders, and roaches. Your loved one probably wont even have any wipes for their rear end to be cleaned, not like the CNAs there will go in there and clean them anyways at least not more then once their entire shift. They lie about their pay you wont get no raise it’s a reason why they “pay for your cna exam” because they’re gonna have you work for them at least 3 months before u even take the exam & once you do no telling when you’ll be on the registry. Nurses ignore your loved ones as soon as you leave. Wouldn’t recommend working here and absolutely wouldn’t recommend having a love one admitted here.
