Horizon Health & Subacute Center

After having a stroke, Horizon Health & Subacute Center brought my dad back from the brink of death. They were the only rehab who thought he had a chance. There were times when I was not happy with the way the staff communicated with him or the family but we powered through. The business office and case managers were great. Most of the hands on staff were too including speech therapy, physical therapy and housekeeping. If you advocate for your loved one, this facility is a good choice.

We transferred from Willow Creek (which was disgusting) to Horizon which was clean, and neat and the people personable and professional. The staff all seems amazing and caring and willing to do whatever it takes to take care of you and manage your pain. The food was amazing and they did everything to accommodate the food preferences and food choices. We are so impressed with Horizon, doctors and staff. We feel blessed to have been accepted here.

I just recently started working here, the nurses are so sweet and nice , the CNAs are so helpful ; the management has made it very easy to communicate with ! I’m very happy and grateful to be apart of the team 😊

My husband was there for approximately 3 months. First of all, the therapists are awesome!! They are very knowledgeable and caring to the residents and their families. I would give them more than 5 stars! The majority of staff is also very responsible from nurses to CNAs, to case workers, and everyone else. The meals for the most part are good with residents having the option to make substitutions if they are not fond of what is on the menu. They also provide activities for the residents and have an ice cream parlor. I just have two criticisms. Apparently upon entering you are assigned to a doctor to oversee your condition, medications, etc. Not sure how that works but I only met his doctor a couple of times and was not impressed. I trusted the nurses opinions much more than his. Second, I think they could use a few more more staff on weekends. If myself or my husband needed this type of service again I would not hesitate to come back here.

Where do I start?! Such an amazing facility and caring staff. I have to say the social workers in this building are very compassionate and care about their residents and families ❤️ My aunt was in this facility and I have to say we are very happy with their care and now she’s home striving with her family. Thank you Horizon!

Amazing staff all around very attentive to residents and families needs have daily activities for residents so their is never a dull moment at horizon will be highly recommending to friends and family

My first visit at this facility was good and was given a tour. This was for my almost 96 year old mom who just had a hip replacement and was taken here. This was a bad choice. During my mom stay she complaint of pain in here hip during the time they were moving here around in the bed. During one of these times the helpers tried to move her and dislocated her hip. They did not realize they had done this, but my mom informed them that she heard a pop and then felt much pain. They waited 3 days before they realized she had a dislocated hip after taking x-ray. She was rushed to the ER on the fourth day and had it replaced. After this incident my mom and family refused to have her go back. I would stay away from this place.

Did a tour, this place smells terrible. 7 months pregnant and looking into rehab for my father. Tour was cut short i couldnt deal with the smell even wearing a face mask, the smell was wretched. Decor is something out of a horror movie. Didn't see any nurses caring for anyone just sitting at the nurses station. Def not a place I want my father. It's a very dark facility. Outside it's beautiful. Inside is horrid. Sad when 90 percent of good reviews are from employees!
