Words can't describe how satisfied we are with the care that my dad received at Heritage Rehabilitation. My dad suffered from a stroke and he went to Heritage after 1-month hospital stay. I want to write the name of every person that was helpful and supportive during our 1 month stay in the facility but this will take up the whole post because it's every single person. It all starts with great management from the coordinator of the facility. The administration employees were very supportive and worked as a team. The registered nurses were knowledgeable. They had medication nurses and wound care nurses. The CNAs who did the direct patient care were truly a gift. My dad was so happy and he called them "angels, not humans". They are so compassionate and caring. We felt that it was a family. We found out on our last day that it was a family-run facility. We felt part of this family. The therapists were very knowledgeable and encouraging. They worked at the level of acute rehab (more specialized rehab services than skilled nursing) or even better. The doctor with was a great advocate for my dad. The social worker with Heritage was helpful. Everyone there was always smiling. Very clean facility as the cleaning crew did their rounds. The food service was great. The bonus was a barber who was there once a week, recreation team with schedule activities to help with mental stumulation, and spiritual representation. Scheduled shower days. Mobility assistance without hesitation. Very compassionate. They have short-term and long-term programs. I have to say that a family member was there first thing in the morning until very late at night, every day. I noticed that they speak kindly with other patients who don't have family with them all the time. They have a call board with lights next to each nurse station. It's run like a hospital. We are so grateful for this gem in the Southbay. My dad will miss these great people and so will we. Keep doing the good work that you're doing and continue being a great support in this difficult time that the family and patient are going through. Thank you!

My mom was admitted to Heritage Rehabilitation Center to recover from a broken hip and spent two weeks at this facility. The staff are kind and caring and supportive. They helped my mom recover and assisted her with rehabilitation and gaining her strength back. Much gratitude for taking good care of her.

My journey from being involved in a near death car accident to finally coming home took two months and three health care facilities. Heritage was the last of the three and I’m grateful. The staff is caring, kind, and responsive. I always felt that my needs were a priority that we’re always competently handled. I highly recommend Heritage.

Grateful for the excellent care that my Mom has been receiving at Heritage Rehab. It takes compassion, discipline and a village and the Administrator and his team has delivered all three consistently for the last 4 years. Thank you!

My mom went there for inpatient physical therapy. Most of the staff was nice, but not very attentive. When she needed help to get to the bathroom they always took at least 15 minutes to get there, and always too late. In the hospital before they gave her a walker and a bedside commode and she was fine, but Heritage wouldn't do that. I watched one of her physical therapy session and it ok. She made progress walking with a walker, but they didn't do much work with her getting out of her bed or a chair (she actually got worse). They discharged her without getting her close to being able to get up and around on her own. I would not take anyone else I cared about there.

So far, I have only went on a walk through tour at the Heritage and found it to be exceptionally clean and they appear to have a good choice of activities to choose from. I am very hopeful to be placed at the Heritage in their long term placement, but without having to be put in with a bunch of Alzheimer's patients, because I am only 59 years old and my mind is still.in tact. I will be calling the Heritage on Monday, 8/13/18 to talk more to the admittance staff about my possibilities of being admitted. My SSDI check is.only about $1675.00/pér month so I am very.hopeful that my check can cover the amount it costs to get admitted.

6 months ago I went to Heritage here for rehabilitation after some back surgery that I had to have which was called a laminectomy and they had me up and walking with a walker in no time at all then they also trained me how to use a cane as well and I was at Heritage for around 10 days or so. I rate Heritage as No. 1 with great nursing as well as a great therapy room with wonderful nurses and therapists who took really great care of me. So if you are looking for a place for excellent Rehabilitation Heritage is the place to go to I highly recommend it. Thank you Heritage for everything. I am also now walking with a cane.

This center needs to be investigated by the state. My mother is currently at this center. My mother is recovering stroke patient and needs maximum assistance. 100% today she fell. The center failed to report this incident. The Director Nestor said so sorry, but I don’t have the report. I said excuse me, you don’t have the incident report when this fall was over 3 hours ago? He said I am sorry... let me contact her nurse. My mother told me about her fall. She also told me that at night she doesn’t get enough rest seems like they are having a party. She calls the nurse for assistance they come all upset and bother she calls for water, and assistance for removal of diaper. She currently has a diaper rash due to her diaper being left for numerous hours. I will follow up with director and demand answers. I can’t wait for the state to investigate.
