Fort Dodge Health and Rehabilitation

I have a loved one there currently amd not only do they bed bugs, that have been spread throughout the building they now have scabies!!!!!! The recent change in leadership has made the building go down hill in my opinion, you feel the tension in the air as soon as you walk into the building. I am currently looking for different placement for my loved one.

Went to see my brother. I live in another state so don't make it back very often. He was covered in sores from a medicine change. No one did anything about it until I showed up. I cried when I saw how bad he was. Another time he is was sitting out with other residents when a woman wouldn't leave him alone. He had told her to more than once when he pushed her away. No staff was there to intervene. I was told by a staff that the woman does that. They were making out to be his fault

Absolutely worst experience. Do not leave your loved one in the care of this nursing home. I am so mad at myself for trusting them with my mother. There were a couple of CNAs that were amazing, but overall unacceptable care and very disappointed in this place.

This place was out dated and dark and dirty. The room window shade would covered up the air conditioner if you pulled it down. I moved the recliner in the room and found what looked like old bacon strips balled up!!! The bed mattress was hard as a rock and lumpy and the controls for raising or lowering the bed would still have the bed moving after you released the button. The call for help button was not working so my mom was without help all night long!!! Unacceptable!!! The Administrator said it was the maker of the call button fault! Take responsibility for your staff and out dated equipment. Never use this place!

Treats everyone who walks through the door like family. Food is amazing, fun activities are held daily and the new therapy room is built perfectly with brand new equipment. Therapy staff makes getting stronger fun and is willing to work with anyone in the community. I would recommend this facility to anyone looking for long term care or rehab.

Was the absolute worst Care Facility that I have ever seen. I was there before Xmas of 2020, contacted COVID-19, had two different patients crawl into my my private room, one looking for a bathroom, the other, his car keys, both times it took close to 15 minutes for any help was received. The food was horrid, cold and soggy being the only thing consistent. Totally inept and understaffed.

My mother had fantastic care at FDHR, and myself and daughters want to say how much we appreciated the great care she received. Thank you also for all the concern and love from the staff we were given during our time of grief.

Facility is clean and adequately staffed; therapy team performs an excellent job getting residence to function and gain independence better than most that I have seen.
