My wife entered this rehab. They never removed her trach tube. They didn't even clean it for four days. They do not respond to patient requests, and do not provide much oversight of the patients. We had to complain multiple times to get them to do their jobs......Trying to get her out of here before it's too late.

One of my relatives was a patient at this facility. When she was transferred to a hospital for medical treatment, the staff at the nursing home were uncooperative when we tried to retrieve the personal belongings. I am still waiting for a call back from the executive director. This is unacceptable.

I have no idea how this place is still operational. This is no place for a human. This is a relative that has been neglected and mistreated for years. She is on a breathing assisting machine and has no mental awareness. It is believed that they are going against her final wishes and she has not had any type of food or liquid naturally consumed since March 2020. The one star is so I can post this. It will not let me otherwise. Should be negative stars. GOD may your Will be done.

I had a wonderful experience here. All the staff were friendly and efficient, I felt very welcomed. All my medication arrived on time. Housekeeping cleaned my room every single day. The rehabilitation staff were phenomenal! Physical therapy and occupational therapy were my favorite parts of the day! Rebecca is amazing! Thank you so much for taking care of me! Highly recommended!

😓I went to visit my dad there on Sept.21,2023 I called first & was transferred to a different phone number. After the phone continually rang & no one answering the number I was transferred to, I walked up to the facility & pushed the button to see if someone would come to answer the door. No receptionist was there at the front desk to greet anyone. Finally, a staff member shows up & after I gave this staff member my dad's name, I was denied access to see him.🤔 I was told he was fine & doing well. I told the staff member that their website says their facility is open 24 hours & expressed to this staff member that I would really like to see my dad. This same staff member said to me, the visiting hours are from 11am - 8pm & I said to the staff member that came to the door, your website says open 24 hours. This staff member tells me that this was a mistake on the website about the hours they're open 🤔 So, I went to the website & showed this staff member exactly where it says their facility is open 24 hours 🤔... The staff member still refused to let me to see my dad reassuring me that he was okay.. The next day on Sept.22, 2023 my dad is deceased... 😰🧐 Now my dad didn't pass away at the nursing home but he did have to be taken from there by ambulance to a hospital because he was coughing a lot & not breathing well when my sister visited him there on Sept. 22, 2023... The staff member my sister spoke to said my dad had been sick. 😱 My sister actually called to have him taken by ambulance to the hospital where he later passed away...😢 The hospital said that he had pneumonia 😥 He was only at the hospital & the nursing home recently. My dad was paralyzed & couldn't care for himself, he was completely dependent on others for assistance & everything he needed. If you have a loved one in these types of places please check on them as frequently as possible & see for yourself that your loved one is either well or not well. Don't just take the word of a staff member there because that could be the last time you see your family member alive, so please check on your loved ones it's extremely important 😥🙏😓🙏😢

DO NOT bring any of your loved ones here!!! I went to an Orientation for a job and a nurse walked behind me and injected me with a flu shot and walked away.... I was there for one day I didn't know her at all.....She did this for no reason!!! I pressed charges against her and they lied for her she was not convicted ... She is still employed there and if she would do this to me? What would she do to the ones who can't speak up for themselves???...If you have a loved one here you Should check on them often and stay on top of any injections or medications they are being prescribed... Furthermore, the home is filthy and had dirty diapers in the hall when I visited...Better safe than sorry.

Do not send a loved one to this Facility. I checked my Mother into this facility for less than 24 hours, she had to be removed because the facility was subpar across the board. - On day 1, there was no formal intake, we just put the patient in bed and eventually had to leave - The patient was moved into a room where a Patient who died prior to entry still had the room decorated with their photos/belongings (it wasn't a memorial, just unremoved items) - On night 1, the patient was overlooked for dinner, and provided a grilled cheese, dietary restrictions had never been asked/recorded - When reaching out to the Healthcare Center to inquire about alternative food options given the oversight, I was rudely hung up on - Despite the patient needing minimal support to go to/from the bathroom, the patient was asked to wear a diaper through the night because the assistance needed couldn't be provided - When retrieving the patient the following day, less than 24 hours in the facility, I observed her roommate laying in her own feces, a clear sign of neglect - My mother was quickly removed from this facility because we observed immediate red flags and signs of neglect, the facility itself is decaying, it reeks of urine, likely because of the patients being left in diapers, please find an alternative; it is not safe.

This is the best center for rehabilitation and physical therapy. The staff here are excellent in what they do. My experience here for the past 8 weeks has been nothing but constant attention and care. Everyone was friendly and sensitive to my needs. Thank you all for helping me walk again. When i arrived, i could not stand, walk, bathe, or clean myself due to a nasty fall that stopped me from walking literally. I could not have gotten through this traumatic event without you all. To the PT team. You are the best. I would definitely recommend this facility to anyone who needed it. I am a success story.
