My elderly mom had a bad experience during a 10 day stay January 2023. The facility failed to follow doctor instructions for PT, failed to address dietary needs, and failed repeatedly to bathe and dress this patient. I moved mom out of this facility when I arrived at 3:45 pm and found her in pajamas and her denture missing (and never found). During her stay, I visited daily. While I was willing to dress her repeatedly or give her a shower, or remind staff about her dietary needs and “no weight bearing” exercise during PT, this was the last straw. My mother spent 10 days here for rehab for broken ankle. During that time, physical therapists took her for walks, etc, ignoring “no weight bearing” instructions from her ortho doctor. Their own discharge form even said “no weight bearing” exercise. At intake, I requested her dietary needs be addressed as she was recently hospitalized after fainting from an episode of irritable bowel syndrome. At no time did their dietician reach out to me, and my written meal plan was ignored even though it was discussed with a staffer and posted. The level of care was poor. This is a corporate owned facility. The staff is pleasant but there’s no continuity from one shift to the next. Maybe they are short staffed. When I found mom in pajamas at 3:45pm, a staffer said she checked mom 2-3x earlier that day. Yet that nursing assistant made no effort to dress mom or find the denture. The discharge nurse rolled her eyes when I asked why the PT didn’t follow the “no weight bearing” instructions. I imagine patients who don’t have daily visitors have it worse. Because the denture was lost my elderly mom (100 years old) has had multiple dental appointments with molds and X-rays being made at a great expense and discomfort. I will be requesting reimbursement from Flatirons. I did request an “exit” discussion with their facility director but she didn’t respond. To Stacie - Executive Director. I’m responding to your reply (below) to my original post. You know exactly what happened. I told many staffers of my on-going concerns including the social worker on your staff and the patient liaison. I asked to meet with you when I decided to move my mother out of Flatirons. To this day, you have never reached out to me. I certainly hope Flatirons will pay for the replacement denture that is being made. Meanwhile, my mother is more limited in what she can eat and her mouth is sunken in. It is no easy task to take a 100 year old to the dentist multiple times in the winter.

While some of the staff are nice and helpful, a few individuals were downright rude. It was actually a shock being spoken to in such a harsh manner simply for asking a question. And loudly saying, "Well, I don't know!" isn't helpful when it comes to finding a solution to a missing walker and cane, or when it comes to returning a phone call. Our main complaints are that there were several thefts of small items, some neglect, and the room wasn't very clean. The staff put dirty clothes back in the closet instead of in a bag or the hamper. The bedside moveable table was often covered in crumbs and spilled liquids, and nobdy bothered to clean my mother's water cup. It was overall an unpleasant experience with a few nicer moments peppered in between. I agree with others who have posted ratings and said that the place seems understaffed, and those there seem unhappy or overworked. Nobody goes out of their way to do basic care like helping someone with both a broken hip and elbow apply lotion. Weirdly, the facility kept asking me to bring more clothing and shoes, but I had already brought an entire closet full of clean clothing and was visiting daily. It seems like the right hand doesn't know what the left is doing, and my mom's discharge was also unorganized. I had to ask for her discharge papers, and nobody offered to help us out. Additionally, there was some confusion about her medications that I had to sort out after we got home. The staff just seemed annoyed anytime I asked a question or for some assistance. I will make every effort to make sure my mom does not go back to this facility. The only reason I'm not giving it one star is because there were a few very kind individuals who were helpful and the physical training staff helped my mom recover, but overall, it was a bad experience and my mom was very often unhappy there.

This facility has been perfect for my Mom, who is recovering from a fall. The PT and OT staff are very knowledgeable in working with someone her age. Her wound care has been exceptional and such a relief for our family. Tara and Caroline and the other nursing staff are so personable, professional and compassionate and that has helped keep my Mom motivated to get well.

Flatirons Rehab has been amazing! My mother has been here about two months now and everything is working out great. The staff here is amazing and they’re really receptive to our needs. Meghan, Eric, Sandy, just to name a few staff members here that I’ve done an amazing job getting my mom back to where she needs to be.

Flatirons health and rehab facility is amazing! My mother-in-law has been there for a couple of months and the nursing staff, PT professionals and administration team are excellent! She’s been very happy there, which has helped in her healing. I would highly recommend this facility.

Best experience ever!! Staff are very nice! & helpful I’m from Louisiana didn’t know what to expect here but working with the staff here was one of my best jobs I ever had. Love it here!! 5 stars. Eric, Binita, tammy are the Best Nurses I ever worked with!! Cece is one of the best CNAs so ask for her by name.

Horrible place. Mom needed discharge from Hospital for rehab. They take her Medicare Advantage. Her secondary is medicaid.They refused her as they were "scared" she would stay beyond 20 days and they wouldn't be paid, as they don't take medicaid. I told them mom is going to hospital for knee replacement in 12 days time, so they won't be out any money. They couldn't "risk it," even though I told them if lighting strikes and mom can't have surgery I will pull her out of your facility at 20 days. They aren't out any money. I am a physician. They refused. Bureaucratic BS. How is this legal!!!!!!!

Flatirons Health and rehab was my home for 25 days. The facility is bright, open and spacious with a homey atmosphere. From admissions to housecleaning, every staff member was cheerful, courteous, respectful, professional .The room was very comfortable with a great bathroom that you could move around in. That's a plus when you have to use a wheel chair. The nurses were terrific Food was good and Justin was awesome in the kitchen. The social coordinator, Megan did a great job handling all the activities always informing us of what was available. With the help of the PT's my goal was met. All of them were very good. Most of my time was spent with Terry who was motivational, encouraging, understanding, humorous and a master at his profession. I was grateful for all he taught me. Flatirons was a very good choice.
