The doctors and nurses are great! Unfortunately, the pharmacy is a complete nightmare! I transferred my medications during a doctor’s appointment on Friday. The following Monday I went to pick up my medications. They were not ready as they should have been. They said my insurance wouldn’t go through! It wasn’t my insurance but, the employees! I left. I made two different calls to my insurance. Both ladies I talked to, reassured me it wasn’t my insurance. Both ladies had called the pharmacy. Both ladies from my insurance said the pharmacy was rude with them and they wouldn’t listen. The last lady filed a grievance. The pharmacy was also rude with me when I called. Later Monday, the pharmacy called and said they figured it out. Three medications were supposed to be delivered Tuesday morning. Well it’s Tuesday and only one was delivered. Needless to say, I transferred my medications back to my original pharmacy. The staff is incompetent, no customer service skills and, just completely the worst in Dayton! Another customer said they never have what you need! Please do not waste your time dealing with this pharmacy! This all transpired within 3 business days! This pharmacy needs to be put out of business! Otherwise, clean house and start fresh with knowledgable, friendly customer service employees. They will NEVER have my business again. I will also tell others to not use this pharmacy! Once again the medical staff is professional and proficient! I would recommend them for your health care needs. Just don’t use the pharmacy!

This center is horrible, the front desk is horrible the nurses are even more horrible and not properly educated! My child was checked here recently they said her impetigo was cleared the nurse said she was fine the very next day we seen a spot! I’ve also needed school physical forms sent over to my children’s school a month ago i called to check where the papers were turns out the doctor never filled the papers out which is causing my children to not be able to return to school until the paper is completed! Avoid this ghetto center if you can it’s a hot mess!

My review is specifically for the pharmacy at this location. I’ve NEVER had any problems, confusion or concerns that could not be resolved or just doesn’t happen. ( 🤞🏽 I won’t in the near future either ). That staff is ALWAYS , courteous & respectful both in-store and via phone. I really appreciate when there is a problem, like billing or refill too soon, etc they communicate this with me. I also appreciate that they allow same day delivery requests after 10:00am; the Miami Chapel location ( definitely the opposite of this one ) however, their cut off time whenever I call in is before 10am per the shortage of drivers…. So they say! 🤦🏽‍♀️ I also want to publicly express my appreciation for Ms. Tabitha ( I think is the name ) for her generosity, patience and understanding on my call in about 10min ago ( it’s 10:24am ). This pharmacy’s team is a great reflection on Five Rivers Health Centers. Thank you 🙏🏼 again for your services and support 🤗

It is NEARLY IMPOSSIBLE to schedule appointments, let alone cancel it or reschedule. Call Center has ONE specific person, who sound young and got nothing but ATTITUDE! I Unfortunately I got unlucky to talk with her about 4-5times and each single time she question you, NEVER ANSWER QUESTION what appointment day available, she doesn't bother to do that, instead she lectures you and if you repeat that you trying to reschedule or schedule appointment, SHE HANGS UP. She hanged up on me 4 times out of 4! And she NEVER schedule me ANY appointment. One time she offered me WALK IN 7.40AM appointment in November TO REFILL my med, that I refused. To my surprise, I got letter in February about missed appointment on that day when I refuse! I had scheduled by CLIINIC appointment on Dec 6 to REFILL MY MEDS, that they CANCELLED WITHOUT INFORMING ME in any form, no voicemail, no text, no letter, no phone call. I found out by accident tru my chart, then I got scheduled 7.40 appointment in November that I specifically refused because it was Walk-in As a result of total unacceptable service I had no asthma and blood pressure meds for A MONTH, and use another clinic out of network to refill my meds. All I got from Five Rivers is letter about missed appointment. Thank you! 0 STARS FOR LACK OF SERVICE and ENDANGERING our HEALTH

I've been attending this practice for at least 17yrs, I really like the new building as the older building across the GSN parking garage was too small. Patients had to sit too close to each other. The new building has plenty of natural lighting as well. It doesn't look dull and dreary like the old building, I would give it 10⭐'s if I could.

I like my dr. But his medical assistant is SO RUDE TO ME! she makes so many smart comments about me and to me & it’s just unbelievable. They always want me here 15 mins early but when I do I’m not seen until later and it’s very frustrating

If you're 10 minutes late for your appointment they might not see you, but if you are on time or early you sit in the waiting room unit at least 15 minutes after your scheduled appointment time, then you wait several minutes for the doctor to come in If your appointment is at 4, that means the front desk is closed after you come out of the office, and they won't schedule you for your next appointment. Very poor patient relations. Also, if the doctor writes a prescription wrong, it's a hassle to get it straightened out.
