Do NOT take your loved one here! My dad was only here less than two days and his condition drastically worsened. He was not given proper attention nor was his blood pressure checked. Thank gosh I got there when I did. I asked the team to check it because he seemed SO much worse than the day before when I was visiting him. He was pale, seemed weak, and he was in lots of stomach pain. Finally, one of the nurses casually strolled over to check his blood pressure, stated that I was right and it was low, and then casually strolled back. It was so aweful. I rushed him back to the hospital. The night team also could not seem to be able to tell if morning team had given meds and exactly what meds were to be given. Some of the staff seemed nice, but I think there was heavy confusion. It was NOT the spot for my dad, and I truly hope the staff checks more on patients and gets to know exactly why each patient is there and what they need.

My grandmother was at this is place because of a stroke and she was discharged a couple days ago and we, as a family could not be happier with the therapy we received the therapist are all wonderful, thank you Paris and Rocio for all you did for us. I will really recommend this place if anyone needs to have great therapy . My only dissatisfaction is with nursing they need to be a little more attentive to their patients.

I have found with my dealings and referring patients to Extended Care they have shown pride and compassion and care to all walks of our community of patients. They take pride in meeting with patients and families to fix any concerns about the care they provide. From the administrative positions to admissions, marketing, business office, medical records, social services, Dir of nursing, therapy to maintenance and activities through dietary. It’s a total team effort that makes this building run as it does. They always strive for improvement.and that’s what it’s all about. 5 stars ⭐️ ⭐️⭐️⭐️⭐️ are hard to come by and they are continually striving to get there and maintain. Thank you for being a partner and helping the community of patients we provide services for.

If you’re reading through reviews either deciding where to send your loved one or learning that they’ve been sent here, THIS IS THE REVIEW TO READ. If I could give this facility ZERO stars I would. My grandmother was placed at Extended Care in early December after falling and breaking her hip. When she arrived at extended care my grandmother was lucid and able to make conversation about current events. Even at the age of 95, she was able to speak for herself and still had life left to her. After sitting in a soiled diaper for six hours her first day here I knew what we were in for. My aunts and I constantly had to search the facility for help and were given responses from “I’m on my break, she’s your mom you do it” to “people are eating on this floor so wait till that’s done and we’ll change her”. I got to the nursing home multiple times to see bruises on my grandma and fear in her eyes. The staff told my aunt she could stay with my grandmother and then returned to kick her out after 12am saying it’s “against the rules”. Funny, I have a recording of a nurse at this facility telling me rules and laws go right over the administrations heads. My grandmother was at extended care a total of two weeks and on the 19th when we brought her home she was a completely different person. She was unable to speak to us normally, feed herself, her short term memory was completely altered … she was deteriorating already. She did a complete 360. We complained about her having a possible UTI for four days before she was released, even spoke to Lori the nurse practitioner who never came back to us again. She told me to my face she would run labs and come back to us with the results so we could get to the bottom of what was changing my grandma so quickly. About three days after she told me that I asked a lady at the front desk about it, and she said Lauri hadn’t run the panels yet, she hadn’t even made a note to have them done. Every time I checked up on it or asked staff to I was told “let me ask someone, I don’t know” or “I can’t help you with that”. She did have a UTI. My grandmother went septic . She passed away January 1st, 2023. My sweet angel grandmother was so horribly neglected and mistreated. Please, for the love of god, do not send anyone you care about here. Also, the picture I included was a bag we found rolled up in my grandmothers dresser in the room. Yes this was there when they admitted her. No they did not apologize. They grabbed the bag, rolled it and threw it away. Let’s not even talk about the sliding door to the patio that’s locked from the outside and wouldn’t close properly . During DECEMBER.

We have received nothing less than great care for our Mom from the entire team and administrators here at Extended care! She has been given the attention that she is accustomed to, from taking care of her personal hygiene, her daily accommodations care, cleanliness of her room, receiving good meals, and she receives the best daily Physical Therapy. We must mention a few names… Jaime, Rosa F., Jovon, Betty, Veronica, Kim, both Administrators, …and the best of the best Hardy & Dani her Physical Therapists. THANK YOU for making our Mom feel at home🙏🏽🙂
