ZERO STARS….I would not recommend this place for anyone. My 46 year old brother was discharged from the hospital with stage 4 cancer to this facility. He was transported via ambulance in the AM, I went to see him several hours later and he was put into a room with no working television and no medication. When I asked about the medications I was told that can take a few days. This is a stage 4 cancer patient at the end of life in severe pain. Thank goodness my brother was 100% competent and was able to talk and function. I can only imagine what would have happened if he was unable to communicate like several other patients. I was able to get the Tv working to help drain out the very loud patients screaming for help in other rooms. I left that evening and received a phone call from my brother in the AM that he waited 30 minutes for help to use the bathroom and no one showed up, he proceeded to try to crawl to the bathroom and puked all over the room before making it to the joined bathroom with another patient. When the staff finally showed up they were obviously mad he got sick and spit gum on his night stand. My brother didn’t even have gum in his room nor did he chew gum. I immediately had him checked out of the facility and transported him via car to another hospital to help get him admitted into a different facility. Unfortunately my brother went into hospice and passed 2 weeks ago. Thank god he was able to communicate and get out of that place, I would never put anyone in this facility. My husband stayed for four days, I was not happy with the place. The help, are very nice, helpful and polite. The section for those with dementia were old, beds, furniture very old, bathrooms out dated. I will not use this place for respite care again. Also, all his sock, underwear, pair of joggers where missed place.

My son (Todd) was staying there for a couple of weeks and everyone was very nice and professional. There is one exception, Susan who is higher up there is totally rude., very short with my son and with me also. As a professional myself I would not have her working for me, especially when the patients in there need kindness and alittle love..

My husband recently stayed there for respite care as he has been bedridden for a long time and needs lots of attention. He received great care and they made sure that he had everything he needed . . and more! A great place to be at a time of need. Would highly recommend Divine Rehabilitation and Skilled Nursing Center.

On Friday March 30th we had an emergency respite for one of our patients. It was also around 5 pm when most managerial staff was already gone for the day. We were in a panic trying to find a long term facility that would accept our patient on such a short notice. Natasha, the admissions director was clutch. She was so helpful and came through for us. We were able to get our patient in to their building and tucked in by 7 oclock. Natasha made sure to stay there the whole time to ensure patient was settled in. Our staff has nothing but positives to say about the staff at Divine and the amazing care they gave our patient. Thank you Natasha and team Divine Sylvania!

My husband has been there for 2 weeks, having him moved. They are under staffed. It takes hours before he's put in a chair, he lays or sits in filth. His light will be on for hours on the weekends. During the week it's an hour & they will come in, shut it off & say they'll be back & don't come. His clothes have disappeared, the housekeeping is bad, the food is poor. He has wounds that should be rebandaged twice daily, they are not, sometimes it's 48 hours before the dressings are changed. I will say that there are a few very good & caring nurses & aids at this facility, but definitely not enough to properly care for our loved ones.

My visit today was very informative. I met with Steve, director of nursing. He was very helpful and open to answering our questions. We feel much better about how to transition our love one from nursing home. We are very grateful. Thank you.
