It was a rough start for my mother at this facility but the staff put me in touch with the Operating Officer of the facility Jordan Clay and he was absolutely amazing! He made me and my family feel heard and addressed our concerns and ensured that our stay was what we expected and we are so grateful for his time and attention! As long as he is over Diamondback then this is the best place for you and your family!

Millie is the most unprofessional person I have ever met. I have worked in many communities of this level of care. I would never have any of my loved ones go to any of the facilities she is a part of. I know personnel who work and work at this facility. I feel so sorry for them, having to work with someone who is so petty and unprofessional. They have lost many amazing personnel because of this person. I will seek other care for my loved ones.

This is one of the worst care facilities I have ever taken my grandmother to. The care she received was extremely negligent, and some of the workers were very unkind towards her. They left her clothes in the closet sitting in her own waste from an accident three days before when I came to visit after they changed her. Not only did they lose her clothes in the wash when they took them, but they also lost her glasses and her phone over the course of a month and a half. We only discovered that her items were missing when we visited her, which we did frequently. If my mother, aunt, brothers, cousins, and I had not been so involved in my grandmother’s life and attentive to the care she was receiving, we would not have known about their unkind attitudes or their lack of care for her belongings. Additionally, my grandmother would push the call button for her nurse, and it would take up to 45 minutes to an hour or longer for them to respond to her needs. Upon visiting the following morning after she was admitted at night, my aunt and I were upset to hear my grandmother tell us that a young lady who came to give her medication didn’t taken to consideration that my grandmother can’t move quickly and seemed disoriented. This young lady told her, "I’m not going to help you if you don’t take this medication." Keep in mind that my grandmother suffers from arthritis, dementia, and Alzheimer’s. Moreover, there was one afternoon when I visited her just before she was getting ready to take a shower. I noticed noticeable red bumps on her neck. When I inquired about them, the staff couldn’t provide any answers and asked if it was the first time I noticed them. I questioned how they hadn’t seen them, considering they were responsible for bathing and showering her. They dismissed my concerns, saying it was "just a bug bite." It turned out that my grandmother had shingles. My family decided to discharge her because we were not satisfied with the care she received. I asked for her social worker multiple times to discuss the treatment she had undergone. The facility lacked communication on all levels. I don’t know how many times I asked to speak to her social worker, and they brought me somebody else. I don’t know how many times I requested her nurse for her pain meds that they took forever in the day to give her. Lastly, I was only able to speak with the social worker and Jordan Clay, when we were leaving. I shared with him the negative attitudes of various nurses, the loss of her belongings, and the fact that they administered her medication without ensuring she took it safely. In response to my concerns, the director simply said, “I’m sorry that’s the care you received.”

My dad has Alzheimer's and Dementia. He was at an assisting living place, unfortunately he fractured a bone and couldn't walk. They recommended us Diamondback Health and Memory Care Center for his therapy and recovery. While we were there, we discovered downstairs is for recovery, but upstairs is for residents. Moving my dad to live at this facility was the best decision. He doesn't speak English and all the caretakers and nurses have made him feel like home, specially Dolores. Dolores goes above and beyond making sure he is doing great. If she sees that he is a little sad, she will make sure he assists to all the activities they do such as playing games, painting, making some art, feeding the humming birds, etc. Unfortunately I cannot take care of my dad every day, but Dolores makes me feel like he is in the best hands possible. Everyone at Diamondback treats the residents with kindness, my dad has been there for a couple of months now and thanks to Dolores and the rest of the team, I've seen my dad improve so much that it makes me happy he is there. Thank you Dolores! and the Diamondback Health and Memory Care Center team.

My mother recently completed 17 days of skilled nursing care at Diamondback Health and Memory care in Phoenix, Arizona, following a series of minor strokes. The entire staff, from the skilled nurses, physical therapists and social workers, was outstanding. The facility was extremely clean and when I first arrived to take a tour, it had the feel of an upscale hotel lobby. The food was good, and our family was always updated on our mother's progress. There were some difficult decisions to be made, but the folks at Diamondback responded to all of our questions with compassion and understanding. Our mother's health and strength improved during her stay. I would recommend Diamondback to anyone who has a loved one in need of skilled nursing care.

I was there for about 5 months and when I arrived I was at another rehab and they told me to expect to never be off of a trach and never be off of dialysis but once I arrived they were able to get me untrached and off of dialysis. With the physical therapy I received I can get from my bed into a wheelchair and into a shower on my own. I came there with 4 major wounds and thanks to the amazing wound nurse's I have almost have them all healed

My grandmothers first night there she suffered an injury during the night and no one documented or called me about. I come first thing in the morning and she has poorly wrapped gauze on her upper arm so I unwrap it to find a huge, open skin tear and a very large open wound that the night care team DID NOT DOCUMENT or even let me know about it. The day nurse had no idea what happened and had zero answers for me. They said “well the night nurse was overwhelmed” That’s not my problem. Then limit your capacity if your staff can’t handle it. This is unacceptable!
