My mother was sent there 10/22, it was clean, very nice nurse Who enjoyed their job?The maintenance in the bedrooms were awesome but i was billed $6346 My mother was sent there again 11/24 Again, the nurses were nice.The rooms weren't as clean and for some reason. The food was awful, I was billed again, this time $ 5828. I guess they don't bill the insurance company.They go after the Patients knowing that they're seniors on a fixed income. And cannot afford this large bill. I am now trying to fight with the insurance company because they didn't bill them. I will never let them send my mother there again. My mother came home sick her doctor put back in the hospital because she was poorly dehydrated, had an infection, and was on antibiotics . Stay away from there.

I have very mixed feelings about Creekside. My mother-in-law was there for several weeks. She said many staff were kind and nice and treated her well. Rehab was good; however, they would miss sessions. I visited her daily to check on her progress and care. The care plan is a computerized plan that is not created specifically for her. Feedback on her progress was non-existent despite my multiple requests to receive it. They say they meet weekly as a group to discuss all aspects of patient care but the family doesn’t receive that feedback. A transition care/social work specialist is supposed to help coordinate all those needs and I was disappointed in how that was handled. I am grateful for the nurses and rehab staff who were kind. I am sad that other staff were rude and unkind in how they do their jobs. I feel awful for family members who are there and not fully aware of their situation. If the facility is close to you and your loved one is fully coherent and there for rehab, then this might be the best location. Just check in daily. If your situation is different, I would explore other options. I would have provided this feedback directly to the center, but I asked several times and was told they were too busy.

I feel sorry for patients who have no other choice but to stay in this facility. Besides it being clean, the level of care is sad. After witnessing how long it took for a call button to get answered after multiple attempts and seeing no one in sight in the hall, on multiple occasions, I never felt comfortable going a full day without someone coming to check on my mother. The first sign that this wasn’t the right place was when I walked in to her waiting on an appointment by the nurse’s station with her head all the way down and slumped in her wheelchair. She wasn’t able to talk very well, had low blood sugar, and it was obvious something was wrong. Three staff behind the desk saw me speaking to her and didn’t even pretend to be concerned. Next, a recent amputee mind you, after experiencing 2 falls in one day (in their care, or lack of) arrived back to her room from the main hospital. For roughly 3 hours laid without a tray after being gone all day, pain meds, or even A/C turned back on in the room when I showed up at 9pm. After questioning what was going on, the nurse said “ I didn’t know she was back, no one told me”. The next day coming in late, I see her crying in pain because the pain med schedule she was supposed to be on somehow got misconstrued to “as needed” and she went the entire day without it because she was not in a state to effectively communicate that. Again, nurse was quick to say he just got on shift an hour ago, to point blame elsewhere. To top it all off, this quickly led to an infection and being admitted back into the main hospital. After a week, I gathered her things from the room since she will not be returning. The staff who recognized me & knew my mother by name helped me locate her things that were boxed up but didn’t manage to even ask how she was doing or if she’d be returning. I sincerely hope for the sake of other patients that playing the blame game stops and higher up staff follow through with what they say. Not just from 9-5 on weekdays. Being short staffed on a weekend isn’t an acceptable excuse. At a restaurant maybe, not when it comes to people depending on you for healthcare.

I really love the staff at this facility. They have all been very sweet and caring to my family member who is residing there. The facility is clean and decorated nicely. My only suggestions would be to hire more staff that can help with patient activities and food quality.

I know every family has a high expectation of how their loved one should be treated, and we were no different. My Dad spent his last days at Creekside and its taken me a little bit to write this review. From the day he was admitted, till his final breath I believe the people at this facility made his transition from this life to the next a comfortable one. Whenever we had questions, or concerns the staff always tried to accommodate our needs. Visiting the facility almost every day for 3 months you get to know the nurses, and I could see the care and respect they had for my Dad and this made things less stressful during a very emotional time. The 4 stars instead of 5 is because I just don't think anyone is perfect. I just know in my opinion, Creekside was a good choice and I appreciate what these people do.

They need more access to different variety of drinks and snacks for patients throughout the day and night. The major difference is the night. Night staff members like Jordi , who are willing to help patients with the smallest and seem like annoying questions, are great. Thanks

Run away from this nightmare as fast as you can. The interior is beautiful, which is how they get you. The staff is incompetent at best and criminally negligent in actuality. Overdosed my mom nearly 5X the amount prescribed. Call lights left on for over an hour countless times. Moms fingers were crushed and toenails ripped off more than once in ridiculously heavy bathroom doors which spring loaded shut. Multiple falls. Left in same clothes, baths skipped, therapy not given, the list goes on. Staff rude and uncaring. This place should be shut down. Just awful. We moved her after numerous attempts to improve her care . Zero stars.

Simply the worst experience we could ever imagine. We came to the hard decision of placing my mother in a nursing home after caring for her for over a year following a very debilitating stroke. The absolute lack of care and downright negligence is criminal. After four short weeks of being there, my mother was rushed by ambulance to the E/R where we were told she was severely dehydrated, she had a urinary tract infection that had developed into sepsis, and she had a blood clot in her lungs (which we were told was most likely from my mother not being turned in the bed). Additionally, the E/R staff informed us that she had been in her own feces (in her briefs) for quite some time and had an extremely bad rash which made it extremely difficult for the E/R team to insert a catheter. The end result is that my mother is now in hospice and is dying. She only has a few days left to be with us. I cannot stress strongly enough how imperative it is that anyone considering a nursing home for a loved one who is unable to assist in their own care look elsewhere. We, as well as family members and friends spoke to the staff on many occasions and we were lied to constantly. We were never told the severity of my mother’s condition. We were the ones who called 911 for an ambulance and when the nurse found out about it she rushed into the room and exclaimed,”you called an ambulance!!” And when we said yes, she stormed out of the room. Again, I urge anyone considering Creekside for a loved one to please reconsider.
