Horrible place. The front desk ladies claim they do not file disability forms and later say yes but with a fee $ The doctor claiming your forms are fake and refuses to sign. That's why he's a doctor and not a surgeon, bc according to him the hospital surgeon was wrong for giving my husband a month off. Don't go to this place, save your energy and time. I'll make sure to report it to the board.
