I wouldn't ever recommend this place. They don't seem to care. To start off my stepfather, a dementia patient, was in here. We were told they would take him and were equipped to handle him. We got him admitted on 3/20 it is currently 5/8. We were told this is a secure facility, it's not. They have 1 door that locks automatically, none of the rest do. It took us weeks to receive his intake paperwork. I was told by an employee that until a few weeks ago they had been leaving the keys to disable the alarms on the fire exits in the door. This facility catheterized him, and when he ripped it out they sent him to Jay hospital. The hospital informed us that in their opinion he should have never had a catheter to begin with. They replaced the catheter and informed us that it had to remain in place for 2-3 weeks while he healed. This location then removed and replaced his catheter multiple times in less than a week. Their reasoning for removing it after the hospital trip was because they "didn't have an order from the hospital to leave it in place". About 24hrs after he ripped it out there was still dried blood on his floor in his room at this facility, they couldn't even be bothered to clean a biohazard. After a week of removing and reinserting a catheter (that they shouldn't have even been messing with) they couldn't get it back in due to severe blood clotting and sent him back to Jay hospital who had to life flight him to Baptist due to the damage done. Several hours after his transport I went to the facility and a bloody catheter and exam gloves were in his trashcan next to the bed in his room. No biohazard container in sight. He has also walked out of this facility multiple times. One of those times we were told that they didn't know how long he had been out and an employee just happened to notice him. I would also like to add that just anyone can walk in. Through my several visits I have never had to check in, no one has ever asked to see my ID or anything. I have always just been waved back. We have thankfully got him moved after being here for just over a month.

This is not a healing place. Some of the people are just too nasty . The people that work in the kitchen don't know how to cook healthy food for your condition. Everyone is served pork every day, even if you are not allowed to have it. And you need armed guards to watch your pain meds. One poor woman screams every morning because she doesn't get the proper dose.

Monday of this past week I went from the hosp to rehab without looiking at reviews, it was close to my home. they were to have a c pap for me to be on, I had penounia and fluid removed from my right l[ung twice, well the worker could not get the mask fitted right, I finAlly sid forget it. I asked about a recliner to sit in and was told my family would have to bring one in. I said no I have lift chair at home. so I sat on rhw edge of the bed to get any sleep. To get help to the bathroom took 25 mins and then was to late, I wear a fall risk band also.I asked for body wipes to clean up and those were not forth coming till lady from social services maybe she brought me some when I showed her fried bm on my left hand, by then I said I was going home. It was against orders ans she told me I would get no help getting my bags out no help geting me to the vehicle, nothing. It was against FL law. I said it was pretty good to get me there, my bags all in and my money then nohelp. I contacted my oldest son at work and he came after work to get me. A very nice lady let us use a wheel chair to get me and things out of the building, could not wait to get out and I do not recommend this faciallty at all. Connie Harris Century FL 7 -14 -2024

If you read some of the prior reviews carefully they seem to be written by employees. As a family member of a current resident this place as a whole not been taking good care of them at all. My family member is somewhat confused after a recent stroke and staff seem to imply that it’s their own fault when they try to get out of bed on their own and fall. They have fallen multiple times during their short stay and we are trying to find another facility. They weirdly do not use bed alarms even for their most confused patients yet they don’t seem to make consistent safety rounds either. The aides who bring the food trays do not make sure the patients can reach their food and rarely assist to eat in hardly any way, not even with opening things. It’s very sad to see these elderly folks struggling so hard to feed themselves soup when the trays are so far out from them. Multiple little things like that add up to an impression that compassion is greatly lacking. Rounds by the charge nurse each shift would be greatly appreciated so that important matters could be discussed but this is rarely if ever the case. The aides seem poorly trained in aiding and assisting patients as well. This place needs better management! Also going to make a plea to anyone reading to encourage people to visit their friends and loved ones in these facilities. A little bit of love goes a long way for them.

If you can help it, don’t EVER send your loved ones here! Most of the workers are just there for a check and nothing more…

Great family environment, started here twenty- three years ago wouldn’t take nothing for my residents who are my family as well. A place with excellent customer service and teamwork as well.
