Canyon Creek Post-Acute

I am writing this on behalf of our mom and family for my 76 yr old mom’s care after her knee replacement surgery June 2024. Our journey started off with her being transferred last minute to a disgustingly dirty and unprofessional rehab facility in Oakland. VERY long story short, I did some research to find a place in Castro Valley, where I live and saw the good reviews for Canyon Creek in CV Our situation became an emergency to get my mom transferred to a safer, cleaner, and more competent place. Leslie, the caseworker was/is the most responsive, caring, professional we ever could have asked for in the time of dire need. We were able to get my mom admitted because of her diligence. Kudos to the rest of the staff! Of course any rehab center, nursing home etc. will never have anything 100% perfect, but this staff truly strived for perfection to the best of their ability. I wish I could name each and every individual staff member to give all praises, but to sum it up, her 2 physical therapists, Sama (sp?) being one, a male another, were absolutely amazing and pushed her in a great way to strive to gain strength to get out of their, she loved both PT’s she had! Her nurses and admin, omg, too many to name all, but Shane, Nadine, Yen, and sooo many more took great care of her. Again I can’t remember all the names of everyone she told me she adored. Debbie, you are an angel that was sent to earth, your skills are above and beyond and we cannot thank you enough! Anna, you were so professional and fought for my mom’s pain management and cannot thank you enough! To all the staff that works behind the scenes cleaning and disinfecting the facility- top notch as this facility gets a A+ on hygiene! In the many times visiting my mom, I witnessed what your staff continually must deal with- people from all walks of life. Many patients with a multitude of issues and scenarios and extremely unpleasant to boot. You all handled it with grace and was clear to the best of your ability. I am a CA insurance agent and did not think there could be a job more challenging currently, but ya’all proved me wrong. A warm and friendly facility! Definitely a great addition to the community for anyone looking for rehabilitation.

This is a beautiful 70 bed facility. I had the opportunity to tour the facility. It was recently updated and has a very modern, new look and feel. Staff were welcoming and inviting. I spoke with the rehabilitation director and they explained their physical, occupational, and speech therapy programs. Would highly recommend this facility for short term rehab! Nice open and large space for rehab with lots of natural light!

This facility has such a great team of leaders and nursing staff! Each patient who comes through the doors is treated with the care and consideration that they deserve. They've also gone through some significant renovations over the last few months. The building looks brand new!

Administrator is very nice and answers all your questions but upon arriving there the place was horrible especially the staff. Al these 5 star reviews I think must be fake. I would not advise anybody to come to this place

My mother recently stayed at Canyon Creek for rehabilitation following a hip fracture. She was very well cared for and employees were friendly and helpful. Highly recommend this facility

This facility just underwent major renovations and is now very modern looking and offering top class rehab services. Excited to be part of the team and growth.
