My friend was in the rehab center for two weeks. She is very grateful to be home! However, she did tell me something I find very disturbing. She told me she saw large roaches crawling on the floor on two different occasions. I really am very upset by this and feel it must be posted lest Bedrock employees are not aware of these unsanitary conditions.

Bedrock Rehabilitation is very unprofessional. I was having my dad transferred from Bedrock Rehabilitation and Nursing Facility in Lake City to another facility here in Jacksonville where I live. I was told by the social worker that everything was confirmed and she asked if I could personally transport my dad to the new facility. When we arrived at the facility in Jacksonville, they had no idea my dad was coming. They had no info regarding his dialysis days and times and we were told that Bedrock hadn’t confirmed the transfer or anything. The new facility went ahead and took him; however, Bedrock did not try to switch his dialysis locations to a facility in Jacksonville, prior to discharging him and he was scheduled for dialysis the next day. Unfortunately, Medicare will not pay for transportation to dialysis in Lake City. Not only that, after calling around in Jacksonville, none of the facilities have any available chairs today for him to get to dialysis, so he has to be transferred out to the hospital. All of this could have been avoided, had people done their jobs correctly.

Bedrock in Lake City Florida is a five-star facility as a resident of bedrock it has certainly changed my life and given me a place I can Feel safe and well taken Care of thank you Bedrock.

staff ignores call lights and sits on their phones all day and pick on patients that can't talk or defend them selves and very poor maintenance showers leak through the wall into the hallway soak the carpet which creates mold never give medicine on time and sometimes it takes them a long time to fix the beds when they break the food is horrible I would definitely not recommend this facility I do not have many choices to do in activities and I'm sorry it is pretty sad when you give gift cards to the employees to get a good review and I was a former resident for a year and a half that got out of there

Great environment! My mom was recently here for rehab and we we're so pleased with the rehab team and nursing staff. She also really enjoyed having her own shower in her room. She was impressed with the activities for her to attend in between therapy. It felt great for her to be in a clean, healthy, and happy environment to get back to her prior levels.

How can you believe any reviews when most are posted by staff (who are compensated with gift cards for leaving reviews) Most people not employed by them have had bad experiences so those are the ones that should be considered the most.

It is pretty sad when you can tell it is shift change because you go outside and all you can smell is weed coming from the cars from the staff that are coming and going and the staff ignores call lights and sits on their phones all day and pick on patients that can't talk or defend them selves the food is not good for someone like myself that is on a special diet because of dialysis and they tell you basically eat what you can that we give you and don't eat what you can't eat and that all the reviews was bought with gift cards for employees to leave five star reviews so it would make it look better for them

I have a friend there that's been there for over a week. He is so unhappy and so are we. He had a stroke and has aphasia. He has had a bladder cystoscopy and has 2 stents in his Kidneys. He has been in his room without a TV all this time, not that we did not ask for someone to assist him. We have had rude comments from the staff and caregivers. He is not getting his meds and when we complain it's: well he has to ask for them. If he did not have 2 people caring for him from the outside he would surely be in trouble. If we had a choice we would never have put him in there. He is not able to take care of himself right now like he should even though he thinks that he is. Waiting on biopsy results to see what lies ahead of him.
