I was a resident for 3 weeks in the rehab department after a botched brain surgery and strokes. Dana in Admissions was amazing and worked beyond her normal hours to get me there from the hospital. I got a nice private room and the best nurses around (much better than the TMH hospital nurses). The therapy area was full of different activities to help strengthen my mobility and the therapists was so nice and made it fun. The only complaint I have is the food and dietary department. They did come and ask me the things I did not like as well as what I could eat in place of the other food. Plus some nights I was able to make an order for the next day on what I wanted. However, I never got what I asked for and I kept getting plates of food that was loaded down with things I would not eat. Other than that I can say having to be here wasn’t as bad as I thought.

You may want to inquire as to what type of grooming they are doing/not doing and how often. This includes combing hair, brushing/flossing teeth, showering, trimming/cleaning nails, etc. Better for residents to have their own skin creams in case they are allergic to the ones supplied. Know that all personal belongings (valuable or not) are at risk of going missing. And having them labeled doesn't always help locate them. In a span of 2 1/2 months, my mother lost quite a few items including her grabber-reacher tool and a wheelchair side-bag. So, less stuff = less risk. You may want to keep on top of upcoming Dr appts with them to make sure they are aware of them and that they're being made timely. You should never see the things I've witnessed; used gloves on the floor, pants that reek of urine on a table, a garbage can that has been full since the night before, bedding that hasn't been changed for days, not noticing personal belongings on a food tray before removing tray from room or, no drinkables on a bedside table for a resident who doesn't drink enough fluids. Some CNAs truly care however and take initiative to help out residents even if they are not assigned to them. But other CNAs are either not trained well or do not care enough. They don't know how to speak nice, have no manners, don't check on residents every 2 hours, don't pay enough attention to residents needs, lack good reporting, and they're on their cell phones/ blue tooth during working hours, or they turn off the call light then leave the room and not address the need until hours later. Both nurses and CNAs need to pay more attention to changes in the resident and then do something about it immediately. If it wasn't for me noticing my mom in bed for 3 days straight and asking her nurse to do something, my mom's health condition would've been far more serious. Also, nurses need to return calls to loved ones and if they say they never received the message, then that is a lie or there is a problem with the messengers.

My step-Mother spent almost 5 years here. Mostly owned by Suwannee Rehab. It is not the best place, but it isn't the worst. I think the staff does the best thay can with what they have. Never enough to go around. BUT they have owed her estate over $300 since last May when she passed away and I can not get any answer as to why they haven/t refunded the money, except that is comes from corporate. Not good business practices! Looking to report to someone.

We were visiting an elderly friend that is in Rehab. It was very depressing walking thru the halls with everyone sitting their wheel chairs in the hallways. It stunk like urine terribly. It was awful. I was glad when we got to the Rehab wing it smelled much better but my friend did not look good. I hope he will get out of that place sooner than later.

My brother who is only 63 was placed there on Dec 1st by Dec 3rd I had to fight the nurses to let him be sent out in an ambulance. I had no way to be there at that moment. I live in KY. He received no medication while he was there. Couldn't get a nurse to even talk to me for an hr and a half. Finally got the ambulance to pick him up. Upon arrival at Lake City Hospital he was already in icu. They got him back on all his meds and iv antibiotic and was then transported to North florida hospital that night at 11pm where he was admitted again as they were the ones that discharged him to the nursing home. Please , please don't send your loved one to this nursing home. Very scary situation we were in. My brother would gave more than likely been dead by the next day. I am reporting this nursing home to the state. If ypu look up their stae reports, you will see how many serious tags and fines they received just in the past 6 months. Shameful and disgusting! Thank God I got my brother out of there! hat moment to elp him.

I'm patient at the Live Oak facility. I came into this place with the greatest expectations and tried to be as nice as possible. And they walked all over me. Getting my medications on time is a continual fight. They don't know how to tell time. The problem is too many patients not enough help. A single nurse cannot tend to 30 people. In essence management is cheap. The food is poor and for the most part the staff are overworked and underpaid. I would not recommend this facility at all. Avoid at all costs.

This is a great place! Everyone is super nice and helpful, and they take really good care of the residents. They are the best, i highly recommend!!!

If I could give a zero star rating I would. This place was under staffed, and they ignored and neglected their patients. I would even go as far to say that I saw boarder line abuse. Some of the nurses were very rude and uncaring. People will hit their call light button and be stuck waiting for usually at least an hour. I saw so many different patients soaked and they had to sit in it and wait for someone to change them. The patients who have family that come visit on a regular basis gets better care then the ones that don’t. The ones that don’t will sit and suffer, and be neglected. The place either needs to be cleared out and staffed with all new, better people, or shut down. I think the state needs to pay them more frequent visits. Please don’t send a loved one to this facility unless you plan to visit every single day.
