Not able to even give zero stars . They failed my father and mother both. My father went there transferring with a slide board and walking with PT , and got discharged home in a hoyer ! Without even properly teaching my mother anything. Not sure what failed them worse, insurance or this facility.

Apple Cocomo cared my mom with compassion before she passed away recently. From the nursing director to, nurses and aids, all staff there are very caring, accommodating and compassionate. I wam very grateful for their excellent care to my mom! Thank you all!

Not a place to send loved ones or people you care about!! .. unless your idea of "good" care for them. Is having them hauled off to the hospital due to lack of care.. only to return to the same lack of care & horrible attitudes.

My mother has been placed there with dementia. It was a difficult decision, but due to the circumstances it was the first available facility. The majority of the staff is good. However there is a strong lack of communication amongst the staff with the family. My mother was taken to an a appointment without my knowledge. The only reason I found out was due to my mother calling me terrified. Also there is nobody available after hours to answer the phones to assist people. Prime example, my mother called me tonight to say another resident was in her room causing a disturbance. This has happened numerous times. My mother rings the nurses bell but yet nobody responds. I call the main number, and shockingly nobody answers the phone. Its getting quite tiring. After reading reviews...apparently coccomo is adequate for short term, however not for long term placement.

First of all let me say that my mother has been at Apple rehabilitation Coccomo a few times as recent as last September. That being said, our last experience July-Aug has been horrible! The communication between staff and family was poor, communication with the Physical Therapist treating my mother attitude was poor and disrespectful, nursing communication poor. My mother had Dr appt with orthopedist. When she came back no orders were carried out. Apparently cna did not bring it back with her. I had to call orthopedic office to fax orders to nursing supervisor. My mother was suppose to be weaned off brace and never was until I called concerned because we already had a discharge plan in place which I then had to cancel. The therapist and I had words about the order not being followed and next day her brace was off! Then a wound was being treated without our knowledge and she was sent home with it. The only way we knew was her live-in aide noticed it. Her visiting nurse had to dress it and mom couldn’t be showered because the wound area was bad and needed to stay dry. Mom also told me she was sick one evening and I tried to call but no one answered phone. Next day my brother went to visit and was stopped by supervisor to ask if Dr called us about mom being sick and lab work and IV fluids were being started. No one called us! Not about dressing wound or her being sick needing treatment! On discharge they lost her brace. She was sent home friday on antibiotic and I was suppose to get report monday with the results of a culture. I had called and spoke with supervisor. I did not get result until Tuesday when I called again to speak with administrator and the call ended so I called back and supervisor answered and then I received results. Administrator knew I wanted to discuss issues but never returned my call so I have placed a call to corporate office and I am waiting for their findings. As of today I have not received a call yet from corporate but he did say it would take time to investigate. This has been a horrible, stressful experience. Please make sure you protect your loved ones. We were lucky enough that mom could be discharged home.

This place is horrible ! My mom was there for a fall. They would leave sheets for her to strip and make her own bed. Never assisted her to the bathroom like the order called for by the doctor. She was in therapy once the whole two weeks she was there. I wouldn’t even let my worse enemy stay here. Staff was polite but did not do what they were supposed to do for her !

My mom is a resident at Cocomo for several years now and I can't thank the staff and the CNA's enough for the wonderful care and compassion that they have shown towards my mom. Cocomo is a top notch facility I could have not asked for a better place to care for my mom.
