My mother in law was admitted there - she was in for 10 days and her health deteriorated to the point that she had to be transported by ambulance and readmitted at Mercy. Both the paramedics and Mercy hospital reportedly turned Apple Creek in for neglect

This nursing home looks nice from the outside and has some nice places inside but please do not be fooled. At least from my experience during a tour, there was a terrible odor and we kept getting rude looks from staff. I was very hopeful that we could find a place to care for my dad but I had a bad "vibe" when I went to visit. Will not be using or recommending based upon my experience.

I checked in to Apple Creek after a series of failed surgeries on my hips. At just 45 I needed help as I was unable to take care of myself or walk. I had just spent the last month at home stuck in bed. I spent two months there. Really nice private room with a roomy private bathroom and walk-in shower. (All rooms are like this). The best memory I WALKED (no cane or anything!) away with is the great staff. The folks there truly cared about me and my recovery. The nurses and CNA’s are attentive and make sure all needs are taken care of immediately. The therapy gym is very well equipped, I have been to many PT places and was glad to see the proper equipment that’s up to date. It was great having an awesome PT and OT team right there to help me with everything from getting up and showered to walking by myself again! I will always remember the great folks at Apple Creek, if I ever need to be in a care facility again in NWA this is my choice by leaps and bounds.

I went to take a preliminary visit for my mother-in-law who is needing care and the facility smells of urine and the staff seemed unprofessional and rude. Did not get a good feeling when I was there and it seemed as if we would be just a room number. We were hoping to find a family atmosphere that felt natural and easy for my mother-in-law but this was definitely not it.

Relocated my Mother & Father In-law here so they would be closer to our home. From the front offices to nursing & transportation. They made the transition very easy for us & parents. The staff from CNA's & aides, to housekeeping, kitchen staff, Activities Director, & more have all been helpful during adjustment period while parents settled in and they truly care about residents & their families. I have popped in on every shift & different days of the week & have had no complaints. Even working with PT, & dietary concerns has been easy. It's such a blessing to have such a beautiful facility with great staff so close to our home. Knowing our parents are minutes away lifted a huge burden from our shoulders & we no longer worry about them because we are always kept in the loop from an accidental skin tear because of Daddy's fragile skin to knowing a mobile Dentist actually comes to Apple Creek to care for the residents on site. Truly is a wonderful home.

Huge thanks to the staff and management team at Apple Creek in Centerton, AR. For the excellent care that you gave my dad, Robert Merritt, during his 6 week rehab for a fractured hip! Diane runs a 5 star health and rehabilitation center. Dad wants to thank Tim and the nurses, Ms. Phillipines, he learned a lot from you, Chris, and so many others! This is a great place to work if a LTC environment is what you are looking for. Great administration, nurses, pay, benefits and facility. They truly care about their patients! The only LTC facility in NWA I would consider sending a loved one to.

I just had to move my mom into a nursing home one of the hardest decisions I have ever had to make but I have to say that I love this place. Everyone is very nice. The facility is very nice. And knowing that the staff cares like I see they do makes it so much easier knowing she will be happy and that I have made the right decision on the location. Thank you Applecreek for your hard work.
